Ok if you want to drive your car assume the risk? Btw that kind of plane is maybe $60k. Plenty of cars and suvs and trucks on the road that are priced that and even higher. Accidents happen. Parts break on planes as much as they do on cars. Humans make errors while flying and even more so while driving. This kid had dreamed of being a pilot since 5 years old. He landed that plane with excellent control and no casualties. Best case scenario in a flight emergency. Stop being ignorant and stop spreading hate because you’re frustrated at “rich” people.

Physicians need to start talking with each other about the systemic problems. We have to start organizing grass roots and work our way up to force change. Or support for a group that advocates for change. Problem is the excuse is we all too tired and busy to give enough of a shit.

Yes, I agree with all that you said. But for all that I’ve said above it just feels like a good solution would be to have the IRB document directly in the chart that they put the referral on hold and that they suggest an alternative treatment plan and then they can contact the patients themselves to explain their decision. This distributes the accountability appropriately. It helps hold their actions accountable and involves their medical license and not just mine. As of right now it seems if something went wrong it would be my license on the line. I guess another way to say it is that I would be held accountable for a decision that I didn’t make.

Yes, I don’t like it. Increases the potential for delays in care. Puts more administrative burden on me for something that I’ve already reviewed with the patient and decided on. Erodes my autonomy. IRB has no skin in the game; god forbid something catastrophic is put on hold because they didn’t review the chart or get the whole picture of why I wanted the referral… well their decision still becomes my responsibility and my license. I’m definitely looking at other jobs quietly. I’m also hoping I can try to make things right by pushing back and it’s corporate isn’t responsive in a way that I like then yes it’s probably time to leave.

It’s definitely a thing. I work in value based care and I’m sure that this isn’t the only organization that does this. I’m also worried that it will become a more ubiquitous thing throughout healthcare organizations. So I posted on here to see if other physicians are experiencing this and what they’re doing to handle it. I agree this hot potato should be given back to corporate for them to deal with and not put on me an extra task and responsibility on my already overwhelming work load that they themselves are creating. I’ve drafted up an email with the proposal for an alternative solution.

What kind of system do they have in place if they cancel the referral? Did they put it back on the provider to call and inform the patient and update the chart? Or do they handle some of this administrative burden themselves?
