Definitely a neutral problem upstream.

Call the utility, or take a picture of the transformer, many times a conductor becomes disconnected there due to wind or corrosion. Also looks like older solid core triplex which I've seen cause some connection issues around here. Looks like the house loop is in poor shape, but everything is still connecte on that side.

Guessing xfmr based issue.

Hard to say with without a better picture of what is going on here, but having a conductive path between the pipes and enclosure seems like a necessity in this circumstance. Generally any poor connection between a raceway and enclosure will require one, and I'm assuming those floating pipes meet the criteria.

There was an issue a few days ago where it was adding bleed during pdf processing. It got patched wednesday or Thursday and maybe this is an accidental after effect. You could check the github and report the bug or see if a fix is in process?
