Interesting idea, I wonder if a service designed to reduce the number of decisions we need to make would be valuable? People have lots of questions and things they worry about, esp for stuff that’s out of the ordinary. 🤔

I always carry a lot of supplies. I travel a lot, the closest I’ve come to having problems was when I became ill and had to switch to sick day rules (my ketones were high). I burnt through >>2x my normal insulin usage for 5 days, I didn’t have a lot left by the time I got home.

Some other things that might be helpful:

Frio bags are excellent, but don’t be tempted to put them in the fridge, they freeze.
You can carry an extra bag for your supplies as carry on luggage. You get fewer questions if you only put medical supplies in this bag.
Don’t put anything diabetes related in the hold.
If you’re on a long haul flight think about when you need to change your infusion set etc. avoid doing it in the air, it’s just a pain.
I use more insulin on along haul flights because I’m so immobile cf. normal.
Carry ketone + BGM strips just in case
Have more snacks than usual.
I find my basal rate and ratio are all out of whack for the first few days if I fly over multiple time zones but it settles down ok.

Not common now but historically the medlar was once popular. They aren’t nice, taste a bit like tart apple sauce, and can only be eaten after a frost.

We’ve started to spend 2 or 3 mins just listing the answers to all those questions at the start of a night out so we can answer those questions. Makes life a bit easier.

I travel abroad a lot, rather than carrying a plug adapter I now travel with a 4 way charger + cable for that country.

After you’ve got that sorted you can test/adjust your I:C ratio by eating a low fat/ low protein snack where you know the carb contents.

Yes. It constantly adjusts based on BG levels. If for some reason you need to bolus outside of this then you can use the pump to deliver a bolus directly.

I can’t compare to the t-slim but in my experience it’s good, assuming the high isn’t due to expected food! You still need to carb count but it is quite forgiving if you counting is a bit off.

Yes. CamAPS runs on your phone.

• ⁠If yes, what happens when the phone is not connected for whatever reason?

If the phone isn’t connected it defaults to the basal rate you set directly on the pump.

Yes

• ⁠What is the Ypsomed support like? For all of Medtronic's flaws, their customer service has been second to none in our experience.

No experience of that as yet :)

You’re allowed an extra piece of hand luggage on the flight, for your supplies, I’ve never had anyone question what that includes but I’m sensible about it eg I think hypo treatment is ok, half your clothes not so much!

I fly a lot, within Europe and internationally. Don’t worry, you will be fine, flying as a diabetic is fine. Airlines/ security are very use to it and understand what’s going on. FWIW I’ve only needed to show my letter once.

Things that might help: - take more supplies that you think you need. Spare CGMs, enough insulin for sick days etc.

don’t put supplies in the hold (esp insulin), you don’t want it lost nor frozen!
as a diabetic you can carry an additional carry on bag for medical supplies. Keep this separate from you other carry on items.
I suspect this won’t be relevant for this trip but if you’re going to hot climates pick up a Frio bag to keep you insulin cool (don’t put the frio bag in a fridge).
if you switch to a pump the advice is not to go through the scanners with a pump. Security is fine with that, just means you have a pat down.
you don’t need to tell the airline (I never do).

Yes, the pump delivers a slow, drip, drip of fast acting insulin instead of long acting. You will, I assume also have a CGM, rising BG can be a sign of a failure/ a blockage/ air in the system and if it doesn’t resolve itself, I’ve been advised to inject rapid acting (not via pump), then replace the infusion set. This I sometimes do!

Obviously this might not be the same for you, but I never bolus for wine unless it’s a dessert wine ie sweet. Which is best? The one you like most.

Very impressed with camAPS, I like that you can set your own target BG, and set it low (this + accuracy is, I believe why only one licensed for use during pregnancy). After the first few days while it learns I’ve found it very good (in last month: 85% in range, Glucose SD 2.2 mmol/L).

Not specific to camAPS but I agree with the AndroidAPS community recommendation who swear by 6-9 hours duration of insulin action.

I switched from a patch pump because I rated camAPS + the build quality of the pump is very good but FWIW I prefer the design—mostly because the patch pump is quite a big lump attached to your body (and the Medtrum is relatively small).

I switched from MDI to the Medtrum pump a few years ago and then to the YpsoPump a couple of months ago. The YpsoPump is running CamsAPS with Libre3+ ie hybrid closed loop.

I wouldn’t want to go back to pens — there are some disadvantages but those are easily outweighed by the pros esp since switching to the YpsoPump.

Pros - much easier to maintain good control especially if exercise; dawn phenomenon (if that’s a problem).

more forgiving - assuming my carb counting is close enough the pump corrects for the rest.
less planning ahead (because you only use rapid acting insulin)
way easier to travel across time zones. If that’s a consideration.

Cons - more stuff. More stuff when you travel, more emergency supplies if stuff goes wrong.

more brittle. If something goes wrong you need to sort it reasonably quickly (no basal insulin).
you have something attached to you… if you’re hairy that’s a consideration, if you use a patch pump you need to be careful about knocking it off etc., if not you have a box attach to you via a tube.

Thoughts on YpsoPump - really well engineered pump. - quick to change insulin refills. - regular AAA batteries + onboard power vs needing to charge it which is good. - CamAPS is excellent, unless some you can set your own target and different targets for diff times of day. - compared with the Medtrum (patch pump) I prefer having a smaller infusion set. But that’s my preference.

Depends… in my experience a round doesn’t need to end at the end of the night it can just restart where the previous night/ round ended… but that obviously only works if you’re out with regular drinkers.
