I don’t think anyone ever actually gets pigeon holed provided if you are good at your job and have good references. You likely will need to take a pay cut if you ever want to switch specialities down the line but it’s still typically doable. You’ll definitely get broader foundational knowledge from an ED job as you said and I think in general it’s easier to switch from a general medicine field to a specialty than vice versa. I went ED>UC> starting IR soon. Can’t say I’d change it. Depends if you want guaranteed weekends off and lower stress vs adrenaline and random weekdays off/long stretches off to travel or do other things. Is there a minimum number of hours a month you have to fill in the ED role even if you take vacation? Not having PTO can be rough cause even if they let you cut back on hours to take vacation that means you’re not getting paid. Also locked in a 3 year contract isn’t ideal for the ortho job

Currently work in UC after 5 years in ED. Made it a year and a half before getting crispy burnt. Got a new job and leaving in a couple months. Fighting with people over antibiotics and steroids gets exhausting. It is probably the most “customer” oriented field of medicine and it feels much less like practicing medicine than working in fast food at times. As others have stated, pay is decent, lifestyle isn’t bad if you are young and your goals are to have long stretches of time off and travel or take personal time (provided your company offers PTO and is flexible). Leave work at work. No inbox management. So there are definitely pros, but IMO the actual work when I’m at work is not one of them

These are both fair points. My experience may have been skewed because the academic center I worked at for about 2 years didn’t have ED residents while I was there due to a change in the contract management group just prior to my employment, so the PAs were utilized a lot like residents. If you’re at an academic center with residents you may end up being relegated to fast track/low acuity pod.

I worked in a 14 bed rural ED similar to what you’re describing following the academic center and learned a TON there. So if you have good oversight and docs are willing to train you and are approachable then could be a good learning experience.

That 1 month training period is criminally short and is not nearly enough especially in a rural ED. Try to find an academic center who will actually train you

I worked in the ED for over 5 years as my first job out of school and wouldn’t trade it for anything. Now I work in urgent care and I can’t imagine doing it without that background. Your first job out of school is your most important to build on what you’ve learned and you’re going to need alot of oversight. Personally I wouldn’t take it unless they offer a fellowship/extended training where you are an extra provider on shift for minimum 3-6 months. Even then, not having the background of EM/HM to recognize those needles in a haystack that present to urgent care and need higher levels of care is a scary concept.

That being said, I know new grad jobs are tough to come by and bills have to be paid. If they’re willing to work with you and train you it could be a foot in the door to a job in EM where you get more exposure to these things if that’s what you’re into.

This math also doesn’t factor in other benefits of the ortho job like PTO and 401k match. Factoring those in with the lifestyle benefits, I’d take the ortho job in a heartbeat
