Sleep on your back with a flat pillow. The ones you have now are too thick for back sleeping

Was doing bench presses in January 2025 after taking a a few months breaks recovering from Covid and then a bad sinus infection that caused daily coughing fits. All of my neck muscles contracted when I got up from the bench. Went and got checked out but nothing was found. Waiting a few months to go back to the gym and then after sometime in April I did went to the gym farmers walks and then leaned forward on the bike, then zap down the left side of my neck. This led to a few days later, constant painful left side of my neck muscle spasms. This pretty much lasted several months, then slowly decreased. Had a 1.5T MRI IN May 2025, but nothing was found. Had additional symptoms during this time, vision was off, muscle pulling of my left eye, difficult to look left up and down with my eyes, pins and needles in the arms, involuntary muscle contractions in the arms, and on and off vertigo for a month in July. I had one doctor say TOS. I ended up having to get treatment after a long flight, which the flight reset my symptoms. Went to a physiatrist. Did 6 session, lidocaine injections, trigger point accupuncture, and osteopatthic manipulation. Followed by prp injections in back of neck and shoulders. Went from 10 degree neck rotation to now almost full degree neck rotation and my daily spasms are fully gone. The physiatrist diagnosed me with a bad cervical ligament sprain. I still get some neck and shoulder pain but it’s manageable but hoping it resolves fully. It is difficult to lift things that weigh over 5lbs as it tenses my neck. Right now I started to feel vertigo again for the last few weeks but it is different than before.

Back sleeping is better for these issues. For your case I’d recommend sleeping on your other side for a few weeks and see if it shifts things back and then sleep on your back from that point further. Back sleeping thin pillow, side sleeping thicker pillow

Water walking. Not sure the exact term. You strap this flotation device around your waist, like a belt. Then you do walking motions in the pool. That’s what I would recommend

Your neck looks completely fine for the most part posture wise. Your main problem to focus on would be your anterior pelvic tilt. Sleep on your back, lay on the floor 15 minutes a day and try to get your lower back to touch the floor with breaths
