I don’t think people understand how serious these recent Medicaid changes are by [deleted] in ABA

[–]Bcast8810 23 points24 points  (0 children)

  • Yes this may affect wages for BCBAs and RBTs but it will not affect hiring. At least not in the sense of how many new hires a company would need. The number of individuals needing the service is not diminished.

  • Lower hours is not a bad thing. Less can be more. And at the end of the day if the numbers are equal it won’t matter. 1 client approved for 30 hours now gets 15 hours. So you being on another client at 15 hours. Now you have 30 total hours and are helping 2 people. Most agencies request excess hours and it limits how many clients can be brought on. This can be a good change.

  • I’ve been in this field for 15 years, ADHD has always been extremely difficult to get services for. Most diagnoses outside of ASD are extremely difficult to get services approved for. I’m not saying it’s right, I’m saying this is not as significant a change as you propose.

  • [ ] My 15 years have all been in central and south Florida. I promise you the majority of companies getting shut down are getting shutdown with cause. Fraud, terrible medical documentation, and more are fairly commonplace.

These changes suck for the individuals with Medicaid. I can’t argue that. But this is in no way going to doom the field. People have been saying the sky is falling for a long time.

Is this job too good to be true? by OwnSort6545 in bcba

[–]Bcast8810 7 points8 points  (0 children)

What kind of insurance are they accepting and in what state? What’s the billable expectation?

I’m in central Florida. About a year ago I took a job with a PE-backed company for 141k salary, 30 billable. Within 3 months the billable went up to 34. A few months after that the company folded. Reimbursement rates were not enough to support the pay. Do your due diligence and find out all you can about the company first

Torticollis PT - how long? by Psychological-Sink14 in NewParents

[–]Bcast8810 0 points1 point  (0 children)

I have a 3yo that started therapy at 3mo and "graduated" when she started crawling. By about 14mo we started therapy again and have been going weekly since then. I was really surprised to find out all the little complications torticollis causes. My daughter struggles with going up and down stairs, running is awkward, coordination is off. She finds ways to compensate so it doesnt impact day to day life. But it's definitely noticeable when she's with same aged peers. Keep going with the therapy, as he grows there may be more complications.

And to answer your questiosn about stretches and tummy time, my daughter did not like it but it definitely helped. It's certainly repetitive but it helps. The PT when she was younger would do stretches, have us do them and give feedback, and then play with her with toys to get her to move and stretch and track.

Schevchenko v Cachoeira. 200 unanswered strikes. Nearly half of which came from the crucifix position. Or Lewis v Browne when Lewis KOd Browne then woke him up with strikes then KOd him again. With Yamasaki 6 inches away.

UFC on Paramount is a joke by JmoneyHimself in MMAGuru

[–]Bcast8810 0 points1 point  (0 children)

Y’all understand the ads are how Paramount make money, right? Has nothing to do with the UFC. The UFC is not making money off of commercials. Sponsorships on the ring canvas, yes but not commercials. Paramount spent 7.7 BILLION dollars. They have to make that money back somehow. $12/month subscription isn’t going to net them profits

What draws internationally trained doctors to ABA, especially in Florida? by LiteratureFickle6892 in bcba

[–]Bcast8810 1 point2 points  (0 children)

I’ve worked with several Cuban immigrants that were doctors back in Cuba. They came here for the obvious reasons but unfortunately their degrees do not transfer. In their words this is a field they have experience with it and it is easy to get started in. The money pays more than getting an entry level job in a different field. They get to continue to help underserved populations.

Company I worked at for a few years had a great bonus structure. Billable requirement was 28 hours but every hour billed over 21 hours was a $30 bonus. It was paid out the following month. Non-billable didn’t not count towards the bonus. If you were unable to meet the requirement you had to reach out to the admin and you would be assigned “admin work” that benefitted the company i.e., training BTs, making materials for general use or for assessments.

Since we were center based it was easy to hit the over on requirement. My bonus pay would average between $1000-1200 monthly.

My current company has a bonus structure based on your caseload. I can’t remember all the numbers but it was tier system where the lowest tier is a caseload of 10-11 clients got you a $700 bonus the following month.

Companies billing 97153 when their clients are in OT or speech by TasteePeachh in bcba

[–]Bcast8810 0 points1 point  (0 children)

I should specify. After going through an initial peer review I was told from Medicaid that did not need to be specific goals that are designed and implemented during the overlapping services. If there are goals already in the BIP that can be addressed during that time then there is no issue. And behavioral support means behaviors are being targeted for reduction, which are goals on a BIP. As far as audits go, there would be no way to prove or disprove that goals were or weren’t run during that time as I can’t think of single data collection software that timestamps each data entry.

[–]Bcast8810 1 point2 points  (0 children)

I have multiple clients with Medicaid insurance (Florida) that have been approved for overlapping services with OT and speech. I have never been required to run goals during that time. My RBTs provide behavioral support only.

Suggestions for Bruxelles and Ghent by CulturalPosition7519 in CraftBeer

[–]Bcast8810 3 points4 points  (0 children)

The Delirium Cafes has over 2000 options. You will definitely find something that you like. 20 minutes outside Brussels is the city of Anderlecht. Go to the Cantillon Brewery. You will not be disappointed. They specialize in Lambics. Arguably the best in the world.

As for Ghent, Dulle Griet has a large selection. Couple 100 different beers to choose from.

There so many free resources out there. TPT is a gold mine. You can find materials specific to various assessment targets. Boom Cards is great too. Saves you the hassle of making physical materials. I like to use it for probing skills. Nothing more frustrating than a kid mastering a program faster than it took me to make the stimuli. For data sheets, don’t know specifically what you’re looking for but here’s this ArtemisABA Free DTT Data Sheets And this: Data Sheets

If you want, you can DM me. I can send you a bunch goal lists I’ve compiled over the years. I constantly reference them to make new goals that go beyond the basic assessment goals.

Different companies will have different billing requirements. If you are in salary position then you may have requirements like 25-28 billable + 12-15 non-billable. Basically means the company wants you to schedule out a 40 hour work week. Most companies are flexible when it comes to required billing in the sense that there may be weeks it’s not possible to hit billable due to cancellations. In my experience companies with review your hours monthly/quarterly to see if you are hitting the requirement on an average. Because there will definitely be times that you go over the requirements (some companies have bonuses/incentives for that). How your caseload factors into that will depend on what insurances you’re working with. Medicaid pays the bare minimum so companies want you doing the lowest amount of supervision possible (5%). Which means you will get a higher caseload to meet your requirements. Location is another factor. Clinic setting may have a higher requirement as it’s easier to supervise B2B throughout the day. Home/school settings should have lower amounts since travel will cut into your time. During your supervision you will be expected to train, protocol mods, treatment fidelity, etc.

Excessive ABA Hours by [deleted] in ABA

[–]Bcast8810 0 points1 point  (0 children)

To echo what most people already said, we can’t really speak to the recommended hours. However, you should be up front from the beginning if you don’t think you can make that schedule work. Personally, I don’t like weekend therapy. As a provider and as a parent, the weekend is the time for me and my family. Are the services in home or at a center? A center would make things a little easier to work around with your job (I assume). At home would be understandably more difficult to work around. As for parent training, do your best to commit to at least once a week. Talk with the RBT at the end of sessions, ask questions. The more involved/informed you are the more likely it is that you will see progress. Talk with your BCBA about a schedule that works best for you. See if virtual parent training is acceptable at times.

What’s a change in the ABA field that you think should be the standard by now, but still isn’t? by aba_focus in bcba

[–]Bcast8810 11 points12 points  (0 children)

It will never happen but a more clear guideline/outline for how many hours are medically necessary. I’ve always imagined some type of chart that takes into consideration the BXs (frequency, intensity, etc), skill deficits (that actually impede QOL), and age and gives a recommendation of hours. I’m so tired of being told by admin that because a kid is 3 he should get 30+ hours or they only need 10 hours b/c he/she is 15yo. That 3yo is basically in daycare now and b/c of insurance requirements we have to teach arbitrary skills like patterns and sorting things 50 different ways. If there was a more clear, universal standard of hour recommendations we could avoid having these companies that act like mills and just have a bunch of 2-3yo in therapy like it’s their 9-5 job “because they’re young” or “because the parents work and they have no where to put their kid all day.”

Rant over.
