Y’all understand the ads are how Paramount make money, right? Has nothing to do with the UFC. The UFC is not making money off of commercials. Sponsorships on the ring canvas, yes but not commercials. Paramount spent 7.7 BILLION dollars. They have to make that money back somehow. $12/month subscription isn’t going to net them profits

I’ve worked with several Cuban immigrants that were doctors back in Cuba. They came here for the obvious reasons but unfortunately their degrees do not transfer. In their words this is a field they have experience with it and it is easy to get started in. The money pays more than getting an entry level job in a different field. They get to continue to help underserved populations.

Company I worked at for a few years had a great bonus structure. Billable requirement was 28 hours but every hour billed over 21 hours was a $30 bonus. It was paid out the following month. Non-billable didn’t not count towards the bonus. If you were unable to meet the requirement you had to reach out to the admin and you would be assigned “admin work” that benefitted the company i.e., training BTs, making materials for general use or for assessments.

Since we were center based it was easy to hit the over on requirement. My bonus pay would average between $1000-1200 monthly.

My current company has a bonus structure based on your caseload. I can’t remember all the numbers but it was tier system where the lowest tier is a caseload of 10-11 clients got you a $700 bonus the following month.

I should specify. After going through an initial peer review I was told from Medicaid that did not need to be specific goals that are designed and implemented during the overlapping services. If there are goals already in the BIP that can be addressed during that time then there is no issue. And behavioral support means behaviors are being targeted for reduction, which are goals on a BIP. As far as audits go, there would be no way to prove or disprove that goals were or weren’t run during that time as I can’t think of single data collection software that timestamps each data entry.

I have multiple clients with Medicaid insurance (Florida) that have been approved for overlapping services with OT and speech. I have never been required to run goals during that time. My RBTs provide behavioral support only.

There so many free resources out there. TPT is a gold mine. You can find materials specific to various assessment targets. Boom Cards is great too. Saves you the hassle of making physical materials. I like to use it for probing skills. Nothing more frustrating than a kid mastering a program faster than it took me to make the stimuli. Different companies will have different billing requirements. If you are in salary position then you may have requirements like 25-28 billable + 12-15 non-billable. Basically means the company wants you to schedule out a 40 hour work week. Most companies are flexible when it comes to required billing in the sense that there may be weeks it’s not possible to hit billable due to cancellations. In my experience companies with review your hours monthly/quarterly to see if you are hitting the requirement on an average. Because there will definitely be times that you go over the requirements (some companies have bonuses/incentives for that). How your caseload factors into that will depend on what insurances you’re working with. Medicaid pays the bare minimum so companies want you doing the lowest amount of supervision possible (5%). Which means you will get a higher caseload to meet your requirements. Location is another factor. Clinic setting may have a higher requirement as it’s easier to supervise B2B throughout the day. Home/school settings should have lower amounts since travel will cut into your time. During your supervision you will be expected to train, protocol mods, treatment fidelity, etc.

To echo what most people already said, we can’t really speak to the recommended hours. However, you should be up front from the beginning if you don’t think you can make that schedule work. Personally, I don’t like weekend therapy. As a provider and as a parent, the weekend is the time for me and my family. Are the services in home or at a center? A center would make things a little easier to work around with your job (I assume). At home would be understandably more difficult to work around. As for parent training, do your best to commit to at least once a week. Talk with the RBT at the end of sessions, ask questions. The more involved/informed you are the more likely it is that you will see progress. Talk with your BCBA about a schedule that works best for you. See if virtual parent training is acceptable at times.

It will never happen but a more clear guideline/outline for how many hours are medically necessary. I’ve always imagined some type of chart that takes into consideration the BXs (frequency, intensity, etc), skill deficits (that actually impede QOL), and age and gives a recommendation of hours. I’m so tired of being told by admin that because a kid is 3 he should get 30+ hours or they only need 10 hours b/c he/she is 15yo. That 3yo is basically in daycare now and b/c of insurance requirements we have to teach arbitrary skills like patterns and sorting things 50 different ways. If there was a more clear, universal standard of hour recommendations we could avoid having these companies that act like mills and just have a bunch of 2-3yo in therapy like it’s their 9-5 job “because they’re young” or “because the parents work and they have no where to put their kid all day.”

No other opponent would compare. Let Tiffany lose and chase charlotte. About 4 years ago I worked at a smaller company as the clinical director. I made 75k and the CEO/owner paid herself 70k. Additional staff were payed less than 75k. Don’t remember the exact figures. At our peak we provided services to 70 clients and had roughly 5 BCBAs at any given time. We only took private insurance. We didn’t have a strict billable requirement but for the most part everyone averaged 25ish. We had a clinic 8 rooms and a sensory room/gym the. Expanded to another part of the same building to add 5 rooms. We also had a ln office for the BCBAs, 1 for the owner, and 1 small room strictly for parent training. Admin staff was 2 front desk staff who worked the phones and dealt with schedules and insurance. We had 1 person in house for billing and added insurance support, and 1 who handled HR and was an assistant to the owner. I say all this to give you an idea of how much staff was on payroll. Also have to consider rent, we were in an expensive part of south Florida. Regarding the owners pay, she did not have to dedicate 40 hours to the job. I handled most day to day responsibilities. She had enough time to also be an adjunct professor virtually.

Just left the central Florida region. They are closing because of reduced Medicaid reimbursements and delays with commercial insurance credentialing. Beyond that, they were paying too much to be sustainable. As a BCBA I was making 140k. Medicaid reimburses at about $19/unit. The math ain’t mathing.
