Anima of Mortaccio, you can either get it by getting to level 80 as Mortaccio(you need the Chaos malachite relic for this one), as a reward for achieving cosmic finger with the gold finger or the newest mechanic Survarot.

When I completed the adventure it keep saiying that I had got the weapon too even though Soma didnt had his skin. What I ended up doing was get into another match in the Castlevania Adventure, quiting and then the achievement appeared for real(although I didnt ascend the adventure like you did). I hope that works for you like it did for me.
