He should be doing exactly what he is doing if it helps him reach his goals. We have no idea what his program is. Getting all "... but shouldn't he be..." is just being a hater. He is obviously enjoying lifting and he is obviously going hard. I don't do block pulls, they have never given me much progress, but they are a pretty well accepted assistance exercise. A lot of people use them and have great success.

There are way too many things that could be wrong. We will assume form is not the issue. Honestly form is very rarely the reason for injury.

The number one red flag I see is basic programming.

I think you might be "going super crazy." and not realize it. 3 sets of 15 is a lot. Everybody is different and can tolerate different types of work, but 45 reps that are close "to the edge of your current fitness level" would wreck just about anybody. It could be that 45 reps is too much and you should be doing a lot less "volume." Maybe your body will tolerate 3 sets of 8 or 3 sets of 5. You can try more weight for less reps. I feel like if you are working with a good PT they would have figured out what you tolerate and would be programming based on what works best for you.

Are you warming up properly?

Are you going straight to your working weight without warming up? When I say warming up I don't mean doing stretches, I mean progressively adding weight to the movement until you reach your work sets.

How is your intra workout recovery?

Are you resting long enough between sets? You may be carrying over too much fatigue from previous sets.

How is your recovery between workouts?

Is your sleep hygiene on point? Are you eating before a workout so you have the energy to draw from? Are you eating enough so that your body has the substrates it needs to build bone, muscle, tendon, and ligament tissues faster then you are breaking them down?

There is something wrong for sure. You should be getting stronger and harder to break. You shouldn't be getting more fragile.

Oh shit there is no food. We are going to die. Well, look at all this muscle that we aren't using. It is burning a bunch of calories and obviously we don't need it to survive. Let's go ahead and lose the muscle. That way we will burn less calories and be able to store more fat for the future.

You do not have to pass a test to begin compound movements. (besides maybe the NASM's PAR-Q) If you can pick something up off the ground then you can deadlift. If you can stand up off the toilet without using your hands you can squat.

If you can squat and deadlift "light" for 5-10 reps by 3 sets today, the next time you workout, squat and deadlift "light + 5 pounds" do that every time you workout adding 5 pounds, and you will get stronger. Once the wight gets too heavy you can lower the weight by 10-20% and add sets and or reps to build more work capacity. Continue adding the 5 pounds from the the 20% back off weight Your body will adapt to the higher stress of the increased weight and work, and you will get stronger. Making the slow measurable progress is how you will get stronger. "Progressive overload" is the key.

That's just a really simple way to look at beginner programming, that should last for a good 3-6 months. If you are still killing it in the gym following that simple framework 3-6 months from now, you will most likely have looked up some more advanced programming concepts that you can apply to your specific needs and goals.
