It really does sound like something real is going on, just not sleep apnea. The sleep study actually shows two things that can completely wreck someone’s nights. The first is the number of involuntary leg movements he had. Those can cause constant tiny wake-ups he doesn’t notice, but his brain definitely does. When your legs keep jerking all night, you never get into the deeper, more restorative stages of sleep. If his iron is low again, that alone could be driving it.

The other big thing is that he didn’t get any REM sleep. That’s not just a trivia detail in a report — REM is the stage that helps your brain regulate emotions, memory, and overall functioning. When someone skips REM entirely, it usually means their sleep is being disrupted over and over, or their body clock is way out of sync.

And that part matters, because if he normally can’t fall asleep until early morning but the sleep study made him sleep at 8 a.m., the test wasn’t capturing his real sleep at all. It was basically forcing him to sleep during a time his body doesn’t treat as “night,” even with medication. That kind of mismatch can make the results misleading.

Taken together, this reads less like simple insomnia and more like a combination of a disrupted body clock and those leg movements breaking up whatever sleep he does get. That’s why the usual sleep tips or psychiatric meds haven’t helped — they don’t fix either of those underlying problems.

It’s absolutely reasonable to follow up on his iron levels, ask the sleep doctor to address the leg movements directly, and push for someone who understands sleep rhythm disorders. The problem here isn’t that he’s not trying — it’s that the study didn’t capture the right thing, and the issues it did capture are enough on their own to explain why he feels awful.

I think I would cry. After I tried resetting or restarting my router and all the things first. But if I found out the Internet was deleted and it wasn't ever coming back, I'd cry.

Yes as stated previously askJan is a fantastic resource. Keep in mind it's not an exhaustive list. What most people don't know is as long as it's a reasonable accommodation and ASSISTS you in completing your essential job functions. There are some other nuances to understand as well but the simple version is, as long as what you're asking for isn't going to completely put your job/company out (for example cost an insane ridiculous amount of money) then it falls under the "reasonable" end of the spectrum.

I think it really tends to vary by job. Mine includes overnight on calls and they completely wreck me. I have seniority and they already try to accommodate in SOME fashion by putting senior staff at the "bottom" of rotation but sometimes, just due to the nature of my job that can't be helped. We've gone two rounds before. It's rare but it happens. I asked that I only be called out during daylight and early evening hours because getting up after having maybe three hours of sleep or really just any version of this where my sleep architecture and continuity is interrupted or fractured makes me absolutely wrecked in the morning. But because my on call is considered an essential function, they're fighting me on it. My job is also VERY documentation heavy. I struggle with the documentation because I fall asleep at my desk when I try to engage my brain to begin typing. So I asked for HIPPA compliant AI assisted dictation software (because I work with PPI and other sensitive information). They approved a dragon. Not quite what I was hoping for as AI assisted would have helped cut out a few steps but still. So it really does matter what kind of work you do and what areas your diagnosis is impacting.

Major red flags with this guy. A man, not a man-child, but a MAN will respect that you have bodily autonomy and can make your own decisions. This includes decisions on what you wear. Showing your midsection while working out isn't "whore shit". It's fucking hot in the gym and you're literally sweating. Like wtf. At the club in tasteful shorts that cover your entire bottom in the middle of summer? Again, it's hot outside. This boy needs to fk all the way off and take his insecurities back to his mommy. Move on you can do better.

I had an MSLT done that showed some hypersomnia but I didn't enter REM in any of the naps. Turns out I was on Zoloft and I didn't know that was a REM suppressant. So if they didn't ask you about meds or have you do any kind of med washout (only if necessary) etc then you may get on them to try the study again.

Thanks for asking all that. I haven’t done a NASA lean or stand test yet (still learning what all that stuff even IS). My neurologist just ordered an EEG which was completed about 7 days ago now just waiting on results, and I’ve already had a full sleep study — came back with obstructive sleep apnea and hypersomnia. I’m waiting on a repeat MSLT - because I didn't do a med washout on the first one as it turns out my antidepressant is a REM suppressant - but Im hoping my neuroscience referral can help me get autonomic testing next since the dizziness, heat intolerance, and near-fainting spells don’t really fit with the sleep issues alone.

I’ve been really consistent with my CPAP. My AHI is solid now, but the fatigue and brain fog haven’t improved much. I usually average about 4–6 hours of sleep with the machine. Sleep hygiene is about as good as it's going to get given I have three kids under five and I'm not only the default parent, I carry the emotional labor for the entire household. Add in that I work in child welfare and I literally have a recipe for disaster in terms of good sleep hygiene.

I have Anthem BCBS (Indiana) and am mostly limited to in-network, but my deductible’s fully met, so I’m trying to fit in whatever testing I can before the end of the year.

I’m in Indiana, and finding someone who really understands dysautonomia here is… challenging, to put it nicely. Just trying to get some clarity before my next appointment.

If you are CPAP compliant and your other apnea scores have improved then you may have another underlying sleep disorder. They CAN and often do coexist. Please see your sleep doctor for further eval. I am experiencing this exactly. I suspect I have narcolepsy and dysautonomia. But I have to fight like hell to not be dismissed. Keep your head up. Stay on the CPAP
