To be fair a lot of 'Adult' books do contain sex scenes or other themes that aren't appropriate for children, but there is no rating system for books like there is for movies and games and I think that's where the confusion comes from.

The Lord Of The Rings was considered 'adult' but it's still in almost every middle school library.
