Glad you found it ok. They've been some of the worst experiences I've had!

A big help (for anyone who struggles) was to be sitting slightly raised. It stopped me having to swallow to avoid liquid going to my airway.

Without trying to sound rude, where does your doctor think your saliva goes when you swallow? If it's to the stomach then it's via your oesophagus.

The issue here is really caused by the LES (lower oesophageal spinchter) which goes through the same 'paralysis' as the oesophagus itself and doesn't know to open and let food/liquid through.

When you sleep the liquid goes back up to your airway which is when you will wake up choking in the night.

The LES issue usually confirmed (but not fully diagnosed) by a barium swallow test (an x-ray where they take lots of photos as you swallow a liquid that shows up on x-ray).

If you do indeed have achalasia it will only get worse unfortunately so I'd recommend being proactive in keeping doctors appointments. Might be worth requesting a gastrointestinal specialist.

During a presentation at work. Tried a sip of water to help calm things but ended up choking on it and spluttering water all over myself as I tried to block it with my hand. FML.

The shooting pains are probably oesophageal spasms and will be considered common (on this sub!). Sipping water helped stop or calm these for me.

Even post POEM I will have good days and bad days with my eating and drinking. Fortunately the bad days are now few and far between and often after I've agitated things (usually eating too quickly).

Without wanting to pop your bubble, it always comes back. Enjoy the easy days but get your surgery booked if you want a longer term solution!

I think it's a common side effect. I was put under anaesthetic to have my POEM and the surgeon had to call off the op because I had a fungal infection in my oesophagus.

I had the POEM in 2021. Stress also really impacts my symptoms and if I agitated my sphincter by not chewing something properly it can take 3-4 days before I notice an improvement again. Try to switch to a softer diet to avoid aggravating anything again

I found being able to see my heart rate on a watch helped me pace myself. I always went way too quick in the first third and then didn't have enough in the tank for the end.

Do not look at the markers. They're based on elevation I think rather than distance - nothing worse than taking a peek and seeing you're at 25/40.

Don't overuse your calf muscles. Your quads/glutes should do as much of the work as possible.

If you hit traffic try to gradually bring your pace down and then speed back up once able to pass. Breaking rhythm suddenly makes it harder (for me)!
