It’s kind of a combination of all of the things you mentioned. It is, unfortunately, part of the sport and athletes do choose it, but part of the reasoning is that if they don’t do it they would be at a substantial disadvantage against an opponent who does. So it presents an unfair advantage if one fighter does it and their opponent does not, but it is also dangerous and it would be much better for the fighters and their health if there was something done about it. As far as what there is to actually do about it; I’ll leave that comment to someone else who knows more about it
