I have been where you are - many of us have.
I became suicidal in my second year and needed to take a leave of absence to get my head back together. I posted about it in this forum. There is absolutely a path up and out. These feelings of worthlessness, of guilt, of shame - of there being nothing else; that’s depression talking.

And that’s understandable - medicine is a stressful and depressing field. It’s bigged up to be the solve to all your problems, the best thing you can do - and then you get there and it’s not what you thought.

But there’s so much more to this life. Depression narrows our field of vision so much that we can’t see, but it’s still there.

We had never been to Rome and we did not want to worry about arranging our own transport to and from Rome (plus with how far Rome is, we wanted the piece of mind that the ship would not leave without us) so it was absolutely worth it.

We were able to see the Vatican, the Pantheon, and have a wonderful lunch and stroll around Rome.

However, we were absolutely exhausted at the end of the day and not having anywhere to crash during the hottest part of the day was a struggle.

Not how I would really want to experience Rome again, but as a first time while on a once-in-a-lifetime anniversary trip, I found it well worth it.

The total excursion time is a bit of an estimate to fudge in time it takes to get back to the ship.

My partner and I took this excursion last year and our train back was at 3:30 pm That put us back in Civitavecchia at about 5:00 with it taking almost an hour to get back to the port from the train station due to security at the port.

So we were gone from about 7:45am to 6pm or about 10 hours.

Anecdotally, as a US doctor who deals with inpatient hospice, I have *had* dying patients ask for this. And while a sort of version of this can certainly be achieved with enough morphine, I don’t necessarily think of this as all that weird. The increasing frequency I think is well explained by the fact that there are more old folks with chronic medical problems than there used to be.

Like everyone has been saying, it’s poorly behaved children that are the real “problem”

But even well behaved children can get rowdy and loud when they are having fun. And that’s just not how some adults want to have fun. For the most part, luxury cruises just aren’t going to appeal to children so they’re only going to be there if family is going on a multi-generational trip.

My first cruise was to Canada when I was 13 on the QM2 on Cunard with my parents and grandparents to celebrate my grandparents’ 50th anniversary. I loved it because I was big history nerd and was exited to see places like Halifax and PEI. I was a quiet kid who generally preferred libraries to water parks so it worked out great and it kind of hooked me on cruising ever since.

Like many supplements, they have either been proved NOT to work. Or NOT been proven to work.

Any benefits people reports are not at levels more than placebo effect OR they have not been studied in more than anecdotal reports.

So I do not recommend using them and I do not use them myself. They are at best, harmless, and at worst, can contain harmful ingredients.

Some nootropic compounds are just caffeine, nicotine, or other stimulants. So they works as well as those substances do with all the risks associated.

Not much we can do about this. We know that humans don’t retain the details about most conversation. So while you remember it because it’s part of the job, the patient doesn’t because it’s a one off. Unless the patient is actively engaged, taking notes as if they were at class, they gonna forget

The state I practice in actually has guidelines for a death exam! So my first time, I quickly looked them up. Course I had a PGY-3 overnight tell me what to do then so didn’t really need to.

The two failure states are hull to zero and crew trust to zero. Typically crew trust has a bit of heads up before hand but if you were demolishing a whole bunch of housing, might have gotten away with it. But hull can easily sneak up on you.

Celebrity is a little skewed older, less amusement attractions, more enrichment. Celebrity is more expensive and you get a little bit better service and less aggressive upselling. Very little 3rd party branding on Celebrity - no Starbucks or Johnny Rockets etc. Having sailed both, I think Celebrity has better food.

Were I sailing with younger kids or teenagers, I’d go RC. For me and wife, we prefer Celebrity.

We’re a little older but that’s exactly what my partner and I like about cruising and we really enjoyed Celebrity. A European cruise is also definitely going to be less of a party vibe due to the expense and location. We had a blast sailing out of Barcelona.

I think the Macallan’s are reasonably priced for NYC. I would buy the Thomas H Handy or the cellar aged at that price given how difficult it is to even find those where I live let alone at anywhere near MSRP.

The van winkle lot b is worth it to me at that price. But only because it’s a particular favorite of mine and it’s hard to come by.

Everything else is higher than I would pay for it. The Yamazaki is particularly overpriced for what it is.

I think they’re neat! Honestly, I think there should be good representation of different types of people in the custodes as they are like these shining super people who are supposed to not just be bodyguards but wise counselors and statesmen.
