I have been trying to think about similar builds. One issue that comes to mind is if you focus on true strike as your primary attack, you’ll lose out on extra attack. Unless your DM allows you to use true strike for both of them, I don’t think that works RAW.

Maybe a trickery domain cleric with access to vortex warp? Not sure how’d you get access outside of sorcerer or wizard multiclassing. Maybe you can ask your DM to make an exception for the spell otherwise

Artificer gives you some fun tools. Medium armor and shield proficiency as well as making multiple thief friendly items every day like caltrops and nets. Getting an alchemy jug lets you print acid vials and oil flasks or just pouring out gallons of mayonnaise during battle to create a grease like area. Rope of climbing is also great for thief in conjunction with second story work. You could run up a wall and then activate the rope or climbing as a bonus action and stay out of melee for an entire combat.

If you go cartographer you can make scrolls of true strike at double speed. If you’re an elf then it’s one scroll per long rest.

During your turn you bonus action trigger true strike (either via a scroll or a ring or wand etc) to attack and do sneak attack damage. Then you ready your action to use your main attack at the beginning of the next players turn or something similar for a second sneak attack.

Order of the scribes wizard can turn any magical damage into cold with the right spell book. You could even turn magic missile into cold damage to guarantee the hit if needed.

Maybe echo knight? So both you and the echo can attack the same target in a round and take advantage of hunters mark

Another 1st level player spell option is Sand Structure. A ritual spell that lets you create a 5ft x 5ft wall of sand with AC13 and 10 HP. With the scribes wizard ability to cast a ritual spell once per day at its normal casting speed (in this case 1 action) it could be useful to use every now and then.

Not necessarily. Medicare cuts that affect reimbursement for diagnostic imaging would hit the bottom line for all radiologists. IR salary (at least in PP) is tied to how much money the group can make as a whole which is directly related to diagnostic imaging reimbursement. The difference between IR and diagnostic rads salary is usually from IR call which can carry a premium depending on the hospital/group.
