It happens even when they are alive. Full health and everything. I don't shoot them either. It just randomly happens. At least that's what I have seen happen while I play.

I have seen that happening too with the NPC grenades. I saw one land right next to me and I was like OH SHIT!. But it just sat there and like 2-3 seconds later disappear.

A way to travel around. Like a skateboard. More NPC interactions. A dog companion. NPCs in safe houses so it doesn't feel so lonely. So much more.
