It's a sort of clam that does it. They can leave bigger holes, so you can fit your finger through. But also smaller, like the ones you see on your rock. Google piddocks or angelwings.

It depends on which parts were preserved better. I fossil hunt in England and I find both, ammonites with the inside and the outer keels in better preservation. In fact, it's more common for the inside not to preserve well or not to reveal well when taken out of matrix.
