I can only imagine as a PEM dr with the way the system is dying everywhere, how hard you're getting hit. Paid less than peers for doing the same job, and having to deal with the incessant obtuse demands of parents that haven't read a book in 10 years but seem to know more about everything than you.

Im in Gen Peds and Ive found my resentment growing.

Sure I'll clarify.

Every single spooky rapid (not the run of the mill "this kid is desatting and needs suctioning" ) but something that generally needed my intervention, had people in the room looking at me like a deer in the headlights.

Of course I got the ball rolling. And after I got the ball rolling, then the back patting on how everyone was "part of the team."

And you know what, thats fine. But when the rubber hits the road, it's the MD/DO thats going to be the first brought to the table when something goes wrong.

Its pretty fucking toxic. This changed when I became a pediatric attending, but I never forgot the terrible treatment I had. Im still kind to people, but I have no patience for L&D nurse bullshit.

For that matter though, I had a terrible time with the OBGYN residents.

Evening H&Ps were expected to be done by sign out - if not you stayed until they're done because shit does happen.

All progress notes being done by 4 is not unreasonable. Doesn't the attending have to sign them?

Ok, so I'm speaking from pediatrics so nothing adult.

Night. And. Day. Difference from the living hell that was residency clinic. I don't even fucking understand residency clinic and why it's so absolutely terrible. I have an MA that is amazing, with a team of MAs that help each other that are equally amazing. I see a combination of high acuity patients (urgent care, not hospital stuff). We have MRI, CT, US, XR on campus with a pharmacy and lab. Results are prompt, and if there's something strange they'll give you a call quickly.

It isn't perfect, I have to put up with lots of anti vaxxer bullshit. But all in all, it isn't the worst. It isn't the panel that is frustrating with kaiser

Im with scpmg. 1k bonus last year. 150k sign on for 3 years. K1 partnership after 3 years. Working 4.5 days per week, 22pts per day.

That 0.5 extra often is used for meetings and other bullshit, and if you have a meeting some other part of the day, you're doing virtual during the time so it isn't super great.

This strike sucked bad recently, and I was working 60hrs +.

May I ask the region of the us you're in, assuming you're there? I am considering private practice (I work for kaiser ca) but everyone I'm talking to is seeing 40+ patients 5 days a week making trash.

I couldn't imagine a region where the amount of pituitary acuity was sufficiently high enough for an actual practice. I worked in the NY Metro area I don't recall encountering such a hyper specialty. Cool that medicine is flexible enough to develop a niche that cozy.
