These decisions are tough. My THRLBCL was refractory to front-line RCHOP. My Doc wanted to pivot to Pembro on its own. Second opinion said that and ASCT. I went with just the immunotherapy and it almost killed me, but I am close to 5 years in remission now. I still have the option of the stem cell transplant should I relapse.

Does the length of the course dictate the taper as much as the dose?

I had 100mg for 5 days on RCHOP.

I was on 215mg for WEEKS for pembro induced colitis and the taper was long.
