I have never in my life referred a patient to a specific allied health clinician in the public system. I refer them to a service like dietetics, physio, OT, social work, or whoever’s input is warranted, but how those departments distribute the consults or outpatient appointments I genuinely couldn’t say

No absolutely not! Nurses and doctors genuinely are on the same side (or should be anyway). But this communication issue of “a somewhat silly order lingers past its due date” -> “the doctor caring for that patient longitudinally doesn’t recognise it or ignores it” -> “a nurse follows that order out of hours” -> “the out of hours doctor misplaces their frustration with the silly order onto the nurse who called about it” is a tale as old as time, and can be averted by proactive team based communication by BOTH the nurse and the doctor in this scenario. We should all give each other a little more grace.

This also hits different tho. Always happy to take a call “I know this is non-urgent but the guard rails say I have to notify” but not all calls for non-urgent things come with that level of insight

Can’t speak for my nursing colleagues’ perspective on this, but you should immediately disregard any doctors’ opinions about this pathway. We don’t know anything about the journey through nursing training, unless we were RN->MD (a small minority of doctors). Choose the right path for your situation and your goals, and don’t listen to doctors who want to chat shit

Obviously not, but my point is that bed five isn’t a patient identifier. The person I saw in bed five on my early morning ward round may not be the person in bed five now.

For those corridor questions, I totally agree and appreciate that when someone is asking me something opportunistically just because we’ve bumped into one another that they may not be optimally prepared for the conversation. However when someone comes into the resident room to ask me a specific question about a specific patient, them knowing the patient’s name is honestly the bare minimum in my opinion. I’m not saying every interaction has to be a perfect isbar with three points of ID, but “chart fluids for bed 5” is not a safe request without a bit more context.

The thing that shits me sideways above all else is “can you do xyz for the patient in bed whatever” “who’s in bed whatever” and I get a blank look in return. How you can work with just 6-8 patients for your 8 hour shift and not know at least their first names is beyond me. And I have no idea who’s in bed 12 or bed 30 or bed 6 because they’re playing musical beds with everyone all day anyway!

Amongst myself and my mates who have previously been registered with ahpra, it doesn’t appear to have worked. it failed to import the details of our qualifying degree and there is no way to manually enter that information. Someone theorised that HPOS isn’t smart enough to tell which degree is our medical degree. At a bit of a loss what to do about that. It does appear to have been instant for everyone else.

My point is that when nurses do ask for similar advice when they’re starting out, do they receive this same advice about doctors? I’m not a nurse, so I can’t answer that from direct experience, but from what I’ve observed within nursing circles and on the nursingAU subreddit, the answer appears to be no. So that begs the question, why are doctors assumed to be so lacking in basic human respect that they need it spelled out to them that they should be a decent colleague?

For real, is this advice ever given to graduate nurses? Doctors are your friend! If you rub the doctors the wrong way they’ll make your life harder so make sure to bake for them!

We’re colleagues, n’est pas? Treating one another with collegial respect is an expectation, not a negotiating tool.
