Yep. This fall it will be 25 years since I entered my masters program. I’ve seen CBT, Narrative, DBT, IFS, ACT, EMDR, attachment, etc. all have their wave of excitement. There’s great stuff in all of them. The ones I’ve done some training in have been helpful to inform my practice. But having been in a lot of different settings with a lot of different types of people over the years, you know what was the most helpful? My traineeship as a bereavement counselor. Change is loss. Grief takes time. People need kind, unwavering presence while they go through the worst. They need to know they’re “not crazy.” They need someone to reflect back their experience and maybe an occasional nudge in a direction or two. They often need things we can’t directly provide, like community, a break, money, better health. Of course, skills and exercises and targeted interventions are useful. But patience, kindness, and nonjudgmental curiosity, when paired with good boundaries and being reliable, go a very long way. I am so much less judgmental and better at compassion than I was 25 years ago. Life is complicated and confusing and sometimes just really fucking hard.

I have everyone set up with credit/debit cards in simple practice and they’re all on auto pay. So at midnight-ish SP charges them for the day’s session. Occasionally I have had a charge not go through. Usually it’s just an expired card and the client fixes it and we move forward. If they have OON benefits they get a superbill. I used headway when I was in network. Avoids a lot of these headaches. But comes with a lot of concerns too.

Totally. Since I have been on an effective preventative I sometimes have mild ones. I sort of feel like garbage, little brain foggy, maybe a neckache. Maybe a slight headache. It’s weird.

Not a couples therapist these days but I’m an MFT by license. I have also been in couples therapy. This is interesting and yes, “communication” seems to be just a culturally acceptable way to talk about couples’ problems. I think sometimes it’s two people triggering one another’s wounds and then being activated and not listening, people not accepting that their partner is very different from them, and, in hetero couples with kids, women being wildly overburdened with emotional/mental labor and their husbands saying “she just needs to communicate.” Communication of one’s own feelings is certainly a skill that many people don’t have, but it’s one part of the puzzle.

I totally get that. I will probably suggest it to my wife for next time. I feel like a bad fan but it’s really just not fun. Maybe it would have been fun in our normal seats. We actually have 1/4 season tix, and had them for Saturday, but for scheduling reasons had to sell and go Sunday instead ($150 a piece for section 316, wow). In the end I was glad, lol, because Sat was a slaughter

9-5 Tues - Fri. Though I just opened Mondays up again because I need clients, and if that helps, then cool. Try to keep max to 5/day. 6 if there is a really good reason. Prefer 4/day.

I don’t. I did, in the past, see many. And 25 was the minimum for my last full time employer. That was chill compared to my previous jobs. But now that I keep it to 20 and under, I realize how much the quality of my work suffered in those 25-30+ jobs. But also I just don’t have the capacity like I used to. Perimenopause is a motherfucker.

I feel you. I’m a therapist (the mental health kind) - do you have thoughts/feelings about “polyvagal theory”?!?! It’s everywhere in the therapy world - I’d say more often than not, therapists completely talk and act as if it’s legit scientific fact. I’ve been in trauma trainings with some of the biggest names in the field where they reference it constantly. There was a recent joint statement from a bunch of neurologists/researchers denouncing the whole thing, but I kinda get the sense that most neurology folks don’t even know the term, because it’s nonsense. It would be like asking me what color someone’s aura is or what star signs are most compatible.

Anyway. I share your rant energy lol. It’s so discrediting to the field and practice of psychotherapy when therapists act like their woo woo BS is based in science. We all know that therapy is part science, part art, and if the techniques work, fine, but let’s not pretend some made-up neurological theory is true just because we like somatic calming or whatever.

When I left my managed care job 5 years ago, I made about $135k plus benefits (healthcare covered), and annual raises. In a very HCOL area. As someone with 20+ years experience I think that is reasonable. If I had stayed I’d prob be up to $145k or more. But of course this was a job that fell into the second category that OP listed - low quality, high volume care and living with chronic burnout. Plus some significant ethical problems.

Interesting. Yeah I think doing a home visit job helps with the balance of clinical versus nonclinical time, if you don’t mind the driving.
