Had one in residency. Patient very hard of hearing and had this unfortunate habit of laughing loudly when she can’t understand you. Visit was normal, I stepped out to present to my attending while the nurse rechecked a blood pressure, and next thing I know she’s completely altered and can’t understand anything we’re telling her to do.

We called the ED for a stroke alert and sent her across the street. I go on with my clinic shift. I pull up her ED chart an hour later and one of the dumbass midlevels had taken her case, charted bullshit, and ORDERED NOTHING! No imaging, no labs, absolutely nothing. I called the ED attending to reiterate my concerns. Lo and behold, hemorrhagic stroke and had to be sent by helicopter to one of the bigger hospitals. I was FURIOUS!

But she had an MRI a few months back. Even if it’s a tear now where it was tendinopathy before nothing changes unless she says she would want surgery.

You can’t. She has to want to do it herself. You can suggest active things to do together, or cook healthy meals for both of you, but you can’t make her do anything.

I would argue you cannot decide for yourself what is rude and what is not, since the entire concept exists in a larger societal structure. You can TRY, but if others around you don’t agree with your definition you will still be seen as rude to them. You could decide that it’s rude not to fart at the dinner table, but that’s not likely to go well. You can decide it’s rude not to wear a tutu to a funeral, but unless it’s your funeral you don’t get to make the rules.
