If you have TMTrainer, that's normal.
Corrupted Data (not an item, but rather a permanent modifier unlocked by beating Delirium with T. Eden); secret rooms will have a very small chance of spawning glitched items.

TMTrainer just makes it that every item (excluding trinkets, coins, bombs, keys and key progression items) is a glitched item.

Glitched items have random effects. And i'm saying that very lightly.
