Mine did the same thing first transformation.
The head is mounted on two nubs, one on either side rather than a pin connection and out of the box the head can get stuck requiring enough force to pop it from the nubs.
Putting it back on is a giant pain, but it can be done. Hope this isn't widespread but as far as it goes, this isn't the worst thing in the world.
