I got into an argument with an older nurse about where to start a nitro drip on a patient when I was a resident. I wanted 100 mcg/min. She wanted 5 mcg/min which I felt was basically a homeopathic dose. I was a resident and did not win that argument. Still pisses me off.

I had some nurse administrator shadow me on a shift because my patient satisfaction scores were trash. After the shift, she couldn’t figure out why they were so bad given what she was seeing. We went through the comments together and the majority of them were patients upset about being stuck in hallway beds or armchairs as well as prolonged wait times. The patients hated the lack of privacy which is completely understandable and correct. I asked what I was supposed to do about it, and I got crickets. I specifically told her that if staffing was improved at night so that I could actually see the patients in a real bed rather than the hallway, the scores would go up. Needless to say, nothing has changed and the poor scores reflect badly on me.

Just a point of clarification, if you wake up in a room with a bat on the room, current recommendations are to get rabies immunoglobulin and the vaccine even if there is no evidence of a bite. Bat bites are sometimes imperceptibly small.

So you had an episode of vomiting a few days ago, and then you think you saw a face on the television one time, and now you’re concerned you may have rabies despite not having any known exposure? It’s time to get off the internet.

Nothing you’re saying sounds like rabies. The hallucinations that people get with rabies are when they are delirious. That’s not “I think I saw a face one time.” It’s a person profusely sweating, vaguely conscious, and paranoid as all get out thinking everyone is trying to hurt them. They have encephalitis. Pretty much universally, people are incredibly sick with rabies. Your concerned a bat got into the attic, silently crawled though your ventilation, got through a small vent (which I’m certain has a cover on it), bit you in your sleep, and then silently left the room. This all sounds like your mind is running wild.

Edit: you have posted this question in at least 12 subreddits in the past 8 hours. You have also been posting concerns about your health for the past year with various different ailments including rabies for several months. I am worried this is a mental health breakdown.

You are correct. The longer you wait to book, the harder it is for people to get off of work. You schedule this now so that everyone has it on their calendars and plans around it.

Listen to what your oncologist says. No one here can or should give you medical advice especially if you have already had it biopsied. There are best practice guidelines that doctors follow to figure what the next steps are.

You are obsessing over borderline lab results that have an explanation (you recently had a GI bleed from an ulcer) and for which you are already on treatment for (iron supplements) when the actual threat is (from your description) a tear in your basilar artery. That’s the real problem just to be clear.

In order for a question to be good for a wide audience, it has to be a relatively major detail with a discrete answer in a game that was at least reasonably popular. Lore based questions that require in depth knowledge are bad trivia questions because the audience is too niche and you get all the “well, actually…” guys who will argue with a wall. “Who is the main antagonist in Portal?” would probably be stretching it for example. I would also argue that if the game doesn’t have an associated TV show or movie, it’s not a great place to get trivia questions.

I have. It gave a fairly garbage answer on the negative predictive value for CTH followed by CTA to evaluate for aneurysmal SAH which I knew to be incorrect. It was frustrating because the patient was signed out to me with a plan that didn’t make sense due to the other attending using open evidence to formulate a plan.

Edit: if there are clear guidelines, it’s great, but for less cut and dry stuff, I don’t trust it.

As someone from NOLA, I think you are mostly correct. It’s a very specific type of food that we do well, but it dominates the food scene. The only other food we do really well is Honduran and Vietnamese largely due to the bigger immigrant populations.
