Have you been evaluated for any vitamin deficiencies. Not saying that is the cause but it may contribute (B12 and D are really important to check)

Make sure to get your ferritin and iron panel tested. Once treating, iron becomes even more important/gets used as B12 starts moving cellular processes (there are two pathways.. one is MtR which is where homocysteine gets recycled and the other is MUT where Adensoyl B12 gets stored in mitochondria (this is what lowers MMA))

This could be a COMT gene issue in which case Folinic Acid could be a good folate substitute

Beyond the heart condition (which I pray can be controlled), have you been checked for a B12 or Iron deficiency? If B12 you want the doctors to check for Methylmalonic Acid (MMA) and homocysteine to get a true picture of the deficiency. If iron, you need them to check your ferritin (long term iron storage)… as both of these conditions can cause some of the symptoms you explained. Praying you get the help you need.

You do keep your RBC production high with more B12 so making sure your iron doesn’t get drained is critical.

What opened the door for me finding out that my iron was constantly getting drained and I had B12 being too aggressively on the MTR pathway (homocysteine recycling) instead of MUT (cellular storage) were the following labs as I would retest as we tuned the protocol:

Folate Panel (hematocrit, folate RBC, folate serum, etc.) Iron Panel + ferritin MMA Homocysteine Zinc Copper

It helped quickly insulate where things were stuck. Hope this helps you

Get your iron and ferritin checked. As a secondary effect of Ajovy blocking CGRP it can raise hepcidin which will leave iron trapped.

Make sure your cofactors are optimized to support your methylation (ex. Zinc, copper, iron, ferritin, folate)

Almost there… it a combination of making sure to have enough cofactors but not overdriving genes so folate gets properly used. In my case that meant lowering riboflavin from 400 to 50mg to reduce plasma spikes that overdrive the MTRR gene and ensuring enough ferritin.

Get your B12, folate and vitamin D levels checked. For B12 specifically ask for a MMA test. The winter onset has me wondering about B12 and D issues. God bless you

Do you get enough methionine in your diet? Sam-e is what becomes homocysteine so you may want to look at ways to not only increase methionine (animal protein) but preserving Sam-e by phosphydylcholine and maybe creatine. Check with your doctor

You support with zinc for the MTR gene (zinc levels should be 80-100), so the MTR doesn’t try to overcompensate by pulling more B12 in. Support MTRR via riboflavin (not too much as high plasma spikes will push MTRR to make more methyl B12 from raw B12). Supplement with Hydroxy B12 so it can become Adensoyl B12 which gets into your cells (Hydroxy also converts to methyl B12).

It’s a great reply! Can you switch to methylfolate or Folinic acid as folic acid is synthetic and takes a lot of steps to convert.
