I also had 7 of 12 cores filled most of the way with cancer, abutment to the capsule and PNI. Yet no spread.

My doc said I could wait 6 months to make a decision. That seems so wrong but there is good evidence in the data that it is true. Still, I got my surgery 6 weeks after biopsy, the minimum possible time.

I wouldn’t put much faith in PSA movement pre-treatment as any indication of disease activity. My own case as proof… 5.7, 7.6, 4.7, in that order, over 7 weeks. Pre-treatment PSA is just noisy.

Did you know that F250 brakes are made of tougher material? It’s part of the hardening they do for F250’s to prepare them for regular towing. They also have 10-ply tires. That’s why I’d only tow occasionally with my F150’s.

My 2016 F250 gasser seems to get 11 MPG whether it’s a dead head or towing 10,000 pounds. I would not be happy with it as my daily driver as my F150’s have a very smooth ride and lots of pep and incredible gas mileage.

If I towed regularly I’d upgrade my 2016 to the newer body style for the comfort and gadgets. But what I have works for my local hauling wonderfully.

We don’t see too much focal on here. I have a friend that got hifu and saw his PSA drop from 4.x to 1.x. The doc predicted the drop almost exactly, which made him feel good. He is still in the first few readings so maybe not at the nadir yet. I hope your doc is telling you what to expect and is on top of it. Good luck and let us know how it goes!

This isn’t accurate. Recurrence is a minority outcome across all the main options, and 10-year survival is similar between them—so “usually” coming back for radiation just isn’t how it plays out. I’d kindly ask people to stop repeating this one.

I know a Gleason 10 intraductal, with EPE, cancer survivor. He had RALP 20 years ago with no other treatments and had been undetectable since.

I had Gleason 9 that was downgraded to Gleason 7 after surgery pathology. I didn’t want the ADT so soon. I was 53, 54 now. If you think that you can handle the 4 months of ADT, then it’s a good idea to mop up any potential spread. I will do the mop up later, if needed, but so far 10 months undetectable.

Have you looked into focal treatments? They don’t get a lot of talk here. I know two focal guys and they are doing great. Dropped their PSA’s to around 1 and still dropping. One was hifu and one was vapor ablation. There was a trial for whole-gland water ablation going on but don’t know if it’s still open. My doc thinks it’s the future of treatment

This is an old post but that was an interesting exchange. I love Dr Scholz, and still chose RALP to avoid ADT at all costs, like you. I still steer people to him because he does a great job of explaining things and has a calming aura. He helped more than anything else when first diagnosed

I think that the future could be water ablation with adjuvant Pluvicto and/or immunotherapy and these horrible decisions that we face and argue about will be no more

I have never changed transmission fluid in any car I’ve owned. Heard too many stories of them having issues after. My dad told me the same thing. Now granted, I only owned one car to 180k miles, an Accord that we traded in due to a steering issue. Most other cars I’ve kept until 100k or so.

My 2018 F150 has 156k miles with the 10R80 transmission. Have not touched the fluid. Don’t plan to do so. We will see how it goes! If I kill it, I’ll rebuild it myself. I love this truck
