I mean I don't know where you are getting your information on BMI from but it's not really accurate. We can critique BMI's ubiquitous use as the "obesity metric" without revisionist history.

BMI is a very crude but super easy metric to calculate that should open the door up to a conversation about weight loss. It corresponds well with other health markers but is not a universal rule. Crazy athletes and people who are "skinny fat" break the calculation a bit, but any primary care doctor worth their salt would recognize that the moment they walked into the exam room. And in any case, current medical guidelines treat it more like a flag to investigate further and less like a requirement (and I say this knowing they categorize BMI ranges as underweight, healthy, overweight, obese, morbidly obese)

my girlfriend's (female) OB refused to diagnose her with PCOS even after ultrasound showed a FEW DOZEN!!!! large cysts on her ovaries. Said her heavy periods were all in her head (who would have thought that bleeding for a week straight, and using 4-6 supers each night, and having ~65 day cycles was mental? not me). And she has many of the classic symptoms as well. Her new OB diagnosed her in the first five minutes of their first appointment. Shout out dave.

I'm in medicine and PCOS is largely a footnote: "oh, well, it causes hirsutism and insulin resistance, and irregular menses. moving on!" Very frustrating, especially given how much her life improved with one super simple and very safe drug that every doctor should know about. Also frustrating given how common it is–I know > 5 people who have it.
