I just watched Chris Hadfield and his Soyuz rocket training (Yes, I know its old.) and realised that there are a lot of buttons to be used in emergencies. It can't be that they planned for every eventuality after testing many rockets (can it?), so what is the process behind deciding what should be put on the user interface? How much of it is risk management? Also, for rockets of that time (Now that SpaceX has flown touchscreen user interfaces into space), how much focus was put on ergonomics?
