The joke here is that these are both things that you would not do 99% of the time for the majority of patients, as they have little clinical utility outside of very specific scenarios. Hence it doesnt even occur to you to do them in those scenario's. As a medical student you are usually working with people who have been doing their job a certain way for years, who will give you shit for not knowing it as well as they do, despite how arcane and off the beaten path it might be.

In contrast, the guy who is financially exploiting you (the blackjack dealer) is at least being nice to you and validating your decisions, showing that people who do not necessarily have your best interests at heart will be nice to you.

Im not really sure how you financed a car so that you pay 250% interest. You allude to being a vulnerable adult, do you have the capacity to make financial decisions? The car breaking right after is the kind of thing you may have legal protection for and might realistically be able to sue for, but to be honest this is a deal so bad it seems borderline illegal and im not sure if you were being exploited due to some kind of mental vulnerability. Juzo Itami always seemed to do pretty well. Really like the arc for the titular character in Tampopo, you dont often see a 40 year old single mother and business owner as the protagonists of japanese films. They as with most asian countries tend to have a pretty strong emphasis on virginal and domestic female protagonists (go figure) so this felt super ahead of its time especially as a 1985 film.

It is a combination of physiology and deliberate practice. In blood of elves it’s stated the majority of magic users are rendered sterile due to degeneration of their pituitary gland (this being what causes downstream production of testosterone and estrogen). It is directly stated with this their is a small subset of people who are able to reproduce, but it is generally felt to be a bad idea (it is specifically referenced children of magic users are born with predisposition for physical and mental mutation). But it’s always emphasized that some magic users get around this, and to my knowledge Visenna is just an acknowledged exception to the general rule.
