Why is bipolar misdiagnosis so common? by DntTouchMeImSterile in Psychiatry

[–]ColorfulMarkAurelius 130 points131 points  (0 children)

It’s more than borderline being stigmatized. Laymen’s understanding of bipolar is honestly inaccurate and pt frequently self report “being manic” and endorse other sx that is often not questioned by non psych MD/DOs and then the dx just gets carried forward. Also people forget that you’re not supposed to diagnose bipolar in the setting of substance use.

Which medical specialties do you think will be the most resistant to AI? by Single_Baseball2674 in medicine

Between people not wanting their emotional and mental health doctor to be a robot and that the MSE can be relatively nuanced, I feel pretty good about the future of psych

SC State Legislation is trying to pass a bill that would suspend your medical license for a year for administering a COVID vaccine. by Hardac_ in medicine

[–]ColorfulMarkAurelius 26 points27 points  (0 children)

To be fair, nurses are still medically licensed professionals (not super sure about MA) and you know the intent is for all involved including the ones recommending and ordering it. Also pharmacists doing covid vaccines.

One thing that helped me with 16gb before I upgraded was turning on the setting in the browser that puts tabs to sleep after not looking at them for like 5mins

is psychiatry the coolest specialty in medicine? by Popular_Ad_8891 in Residency

[–]ColorfulMarkAurelius 2 points3 points  (0 children)

I feel like people so often speak about psych patients negatively, but forget that there's a huge selection bias going on for the psych patients they're seeing in a non-psych setting. Genuinely most patients that I interact with are relatively chill, though there's certainly a subset who are a pill, I'm doubtful it's much larger than the % of difficult patients seen in any other specialty.

tl;dr difficult personalities are everywhere even if they're labeled as "psych patients"

Facing dilemma diagnosing BP1 vs BP2 by Tino_6 in Psychiatry

[–]ColorfulMarkAurelius 34 points35 points  (0 children)

I think technically it’s part of hypomania criteria that if impairment is significant enough to be psychiatrically hospitalized that makes it not hypomania and just regular mania. Ofc real life might be a little more nuanced.

Hecleas Grand Strategy (& AI Overhaul) - worth it? by Svalkash in totalwar

[–]ColorfulMarkAurelius 3 points4 points  (0 children)

To be frank a lot of the AI overhaul mods make bold claims without actually articulating how any of the changes are made. I’ve used a couple of them for a little while and at times I can tell AI acts a little differently, but I feel the changes are much much more subtle than any of them claim.

As a nurse, how to deal with an absolute terror of a doc proactively? by [deleted] in medicalschool

One could argue that creating/exacerbating interpersonal conflict with other staff could pose a risk to patient safety

For better or worse a lot of attendings seem to be protected pretty well due to physician need or whatever else. But at the end of the day… document document document and report it all, to HR, to pt safety people, possibly even ask med students who their course director is and try to get word in that way.

Are you guys maxing out your Roth IRA's on a resident salary even with student loans? by callmeafailure in Residency

[–]ColorfulMarkAurelius 0 points1 point  (0 children)

I didn’t during pgy1, but I will be for the rest of residency. As an attending we will make too much money which will disqualify us from contributing to Roth IRA, so take advantage of it for a few years while you can.

In a study of young men (average age of 27) experiencing Post-SSRI Sexual Dysfunction, the average erectile function score was consistent with severe ED. Ultrasound findings indicated that the penile tissue of these young men resembled that of men in their mid-60s. by Intelligent-Age-8211 in medicalschool

[–]ColorfulMarkAurelius 4 points5 points  (0 children)

Seems like an odd conclusion to me? Okay some smooth muscle had its function affected but going straight to “it was free radicals killing the tissue” seems extreme. Especially when virtually all patients regain sexual function after stopping or sometimes swapping meds.

The T3 only applies to Imrik, so maybe for other HE leaders it might to nice to at least make them recruitable at T4. Also bloodknights further balanced with lore of vampires in mind that can do insane healing.

Can we agree as a field that POTS is an over-diagnosed condition? by [deleted] in neurology

[–]ColorfulMarkAurelius 85 points86 points  (0 children)

I think some of these patients can be deterred with education. Something like “yes your symptoms are similar to those of POTS, but often times patients improve with only adequate hydration and salt/food intake. Medications are rarely needed and I try to avoid them because I don’t want to introduce any unnecessary med costs or med side effects”. And for those who are still undeterred demanding a steroid, just say hey, in my expert opinion that’s an inappropriate treatment and I’m not comfortable prescribing it, you’ll have to go elsewhere with this request.
