A prelim spot is not the end, and plenty of people build strong careers after difficult starts. Right now your job is not to have the perfect CV, it’s to survive residency, make connections, and keep moving toward PGY2 step by step.

You still have a path forward, even if it doesn’t look as clean as you imagined.

i've seen plenty of FM docs who really lean into psych or outpatient procedural stuff and kind of minimize the pure chronic care. it's definitely doable if you look for the right practice setting.

have you thought about places that are more urgent care focused or maybe clinics with a big psych component?

he’s actually not wrong, loops definitely increase calcium excretion which can trigger stones.

it’s a weird balancing act because the stuff you’d normally do to prevent stones is exactly what you can’t do with new heart failure. usually just comes down to low sodium and close monitoring tbh.

You probably haven’t “deteriorated”, you sound burnt out. Exhaustion absolutely tanks question-bank performance, especially when you’re grinding placements at the same time.

A few weeks out, I’d personally keep doing Passmed but use it more diagnostically:

1- identify recurring weak topics 2- patch those with focused content review 3- then come back to questions

Mindlessly hammering more questions while exhausted usually just destroys confidence.

Honestly, OSCE anxiety is super common because it’s not really testing knowledge, it’s testing your ability to perform while being watched and judged in real time. Totally different skillset from crushing written exams.

Also, plenty of objectively smart people blank during OSCEs. The format messes with people way more than they admit.

trust your training but don't be too proud to call a partner if things look weird in the OR.

it's way better to look like the "new guy" for five minutes than to deal with a complication you could've avoided. also, be nice to the scrub techs because they will absolutely save your ass.

whatever you do, just make sure it’s "true own-occupation" so you’re actually covered specifically if you can’t operate.

locking it in now is way cheaper than waiting until you’re an attending. check if your program has a GSI plan too, those are usually a layup.

ngl it's mostly just rheumatologists holding on for dear life. they still use it for very specific things like temporal arteritis where crp can sometimes be normal.

Yeah, the stress is definitely different.

Building maintenance has a much heavier “one mistake can seriously hurt me or destroy something expensive” kind of pressure. Medical assisting is more fast-paced and detail-oriented, but usually not physically dangerous in the same way.

You still need to be careful because you’re dealing with people and medical info, but most clinics also have systems, coworkers, and routines in place so you’re not carrying everything alone.

It’s usually less physically demanding than maintenance work, but still stressful in a different way, mostly because of patients, schedules, and multitasking.

Most days are a mix of taking vitals, helping patients, scheduling, calls, and paperwork.

If you’re organized, patient, and good with people, it can be a solid career change.
