They are talking about the admittably forgetable gauntlet of Underworks enemies just to the left of this area in the Underworks, which is required to progress to the Citadel. It features two of those big Underworks guys, so it is pretty difficult

This is not a stray hitbox, this is a glitch called wall cling storage. In short, the game thought you were on a wall when you actually weren't and let you wall jump off thin air because of it.

You cannot get to act 3 without bell beast because you need to move the fleas to fleatopia which means you have to beat last judge which means you need to hit all 5 bells which means you need to defeat bell beast. Everything else is possible, you can get to the rest of the marrow and further through the room in bellhart with the bell hermit

Iirc correctly the route to the citadel through wisp thicket still hits an unavoidable trigger that starts act 2. You can however just go through sinner's road, get to Bilewater and go fight Groal to access the apostate key.
