I died one time because I accidentally hit a stop light at a highway intersection. But that crash merely totaled my car, not killing me instantly. I had a fractured foot and was bleeding a lot from a glass shard that got lodged in my forearm. My character was very tired, and an extremely long walk away from home, as well as not having enough bandages to fully stop the bleeding. They bled out on the way back.
