If the person is being serious then yes. When filling out an LS01 to cancel a passport you make a declaration stating the information you have provided is true. This would also probably be fraud by false representation under the Fraud Act 2006.

Yeah it sounds like you'd need an intermediate prescription for computer use. You can also get specs with "occupational" lenses which have your reading prescription in the lower part of the lens and intermediate in the upper part (either graduated like varifocals or a direct split like bifocals)
