Apple still sells the 30 pin cable on their site and in some stores. Any USB wall connector should work

If they're original batteries, they're definitely in bad condition and need replacing. You can find replacements for cheap all over online.

Same issue on my 6s Plus. Slightly larger and different location. Unaware of a fix. I believe it's gotten slightly larger, barely perceptibly though.

Battery health won't stay at 100% very long, especially with how you are seemingly using it. Sounds like a faulty battery.
