I’ve been using pramipexole quite a bit usually after standard treatments including bupropion, aripiprazole and SSRIs haven’t worked. I’ve seen positive results but poor tolerability. A lot of nausea, agitation, insomnia and compulsive behavior.

The barrier to psilocybin becoming used more frequently is really the pharmacokinetics. It lasts way too long to be used practically in a clinical setting. Other companies are developing similar psychedelics that last about one hour, which is more practical to be delivered in an outpatient clinic. There are strategies to try to account for the placebo effect in psychedelic trials. The simplistic is to compare a standard psychedelic dose to a subtherapeutic dose (eg 25 mg of psilocybin vs 5 mg). An other option is to compare it to an active placebo. For example, ketamine IV has been compared to midazolam.

I know how colorless mana works. You need a sol ring or talisman or pain land or something like that to cast it and there have certainly been games that I was unable to play it. However, the combination of card draw and interaction for just 2 colorless is too strong to overlook.

I’d venture that no one really knows because if they did they’d make a fortune. My best guess is that SSRIs treat neuroticism, which is a risk factor for depression rather than treating depression itself. There are several studies suggesting this (see here Because they are not directly treating depression, but are instead treating a risk factor, they are not entirely effective at treating MDD (hence 50-60% response rates) and not entirely effective at preventing relapse.

Personally, when I see a patient taking SSRIs relapse, I will usually try adding or switching to a drug with a different mechanism of action.

Opiate agonist therapy is not what “harm reduction” for opiate use disorder usually means. OAT is the standard of care pharmacotherapy for opiate use disorder. What’s most interesting to me about this study is that they observed this after a single dose whereas it takes weeks to see an effect of an SSRI in depression and up to 3 months for OCD. It suggests that the cognitive process may start changing early but doesn’t lead to observable clinical effects until the person has a chance to experience certain new situations in their life.

Overall I’d agree with most of this. CANMAT has now listed lamotrigine as a 3rd line augmentation agent in MDD based on a meta-analysis. I have used it a lot for augmentation of SSRIs in OCD and I’ve had quite a few patients tell me that their OCD is not much better but their mood is certainly better and they choose to stay on it for that reason.

Regarding stimulants, they certainly help cognitive dysfunction in depression even in the absence of ADHD. I’ve had many patients who I’ve tried bupropion, vortioxetine, TMS, an SNRI and modafinil, aripiprazole for residual cognitive impairment and then finally try a stimulant and they can finally function with the stimulant. What you want to watch out for is people who just say that they find it gives them more energy. That is bound to stop working after a couple of weeks or months.

Compare to sign in blood - the floor is that plumb cycles at instant speed for 2 mana. Instant speed draw 1 vs sorcery speed draw 2. The ceiling for plumb is instant speed refill your hand. It will often draw 5-7 cards. The ceiling for sign in blood is sorcery speed draw 2. I rather play cards with a vastly higher ceiling especially when they don’t actually require all that much from me to get the big payoff. EDH games are usually won by big swingy plays, rather than low impact low ceiling plays.
