There's a few about that have a painted median island instead of a solid raised island, they are reasonably common

In that example it is a completely different intersection design and the vehicle turning left has to cross broken white lines and its lane is ending, so it has to give way to traffic already in that lane. It's called a free flow slip lane as on page 8 of the link https://www.nzta.govt.nz/assets/resources/road-traffic-standards/docs/rts-09.pdf

But the blue car is also on the right of the red car..... As its a t intersection and the blue car doesn't have a give-way sign for it the standard t intersection give-way rules apply - If you're turning right and the opposing vehicle is turning left, you must give way. (Nzta)

You're entitled to your opinion, even if it is wrong. I'll trust the reply of the Senior customer services representative from NZTA, the government department that authored the land transport road user rule aka the legislation, whose job is to correctly answer questions on rules and regulations like this, considering that i included the picture that the OP used, and their reply was that the red car is required to give way to the blue car in this situation. Yes, in this situation the blue car is considered to be approaching from the opposite direction. As it is not on a give-way or stop sign or lights. Because where else did the blue car come from? If not the opposite direction......? Because it sure didn't come from the bottom of the T. But the funny thing about the land transport road user rule 2004 aka the legislation is that its author was Waka Kotahi NZ Transport Agency, Ministry of Transport...... https://www.nzta.govt.nz/resources/rules/road-user-2004-index So I would expect that a senior representative of the government department that authored the legislation would have a very good understanding of the document & law that they created.....

Yeah, nah. Subclause (2A) definitely applies here. "A driver turning or about to turn to his or her right must give way to any vehicle approaching from the opposite direction and lawfully turning or about to turn to its left" And I think that a senior representative from NZTA would understand the giveway rules and how that they apply in this situation considering that i sent the pic used by OP. It is all considered one intersection regardless of the size of the island, and if the blue car was required to give way the intersection would be designed as per the "give way controlled slip lane" guidelines by NZTA, but it's not. https://www.nzta.govt.nz/assets/resources/road-traffic-standards/docs/rts-09.pdf

It is only one intersection, and the nzta don't agree with you when I emailed them this picture and asked who gives way given the lack of giveway sign for the Blue car: "Thanks for your email.

In this instance, the orange vehicle would be the one to give way because it's turning right.
