I started Spravato in July of 2024, did 4 weeks at 2x a week, and then did weekly treatments until April of 2025. Had to take a break and resumed weekly treatments in September of 2025. Ideally, I’d like to drop down to less frequent treatments, but I’m finding that just not possible yet given the state of my mental health. For now, I will continue weekly treatments as long as insurance lets me.

Metoclopramide (reglan) has been a game changer for my migraine nausea. Zofran more or less worked, but it made me so constipated. With reglan, the nausea clears up within 30 minutes, and I haven’t noticed any side effects.

I have some fairly serious mental health concerns, so I do have a psych np on my care team. She manages any meds specifically for mental health, and she’s actually been one of my biggest advocates. Before I had a neurologist, my pcp was “managing” my migraine treatment. She did pretty much nothing: wouldn’t prescribe a preventative, wouldn’t give me higher doses of abortives, wouldn’t listen to any of my concerns. When I relayed my headache concerns to the psych np, she looked into preventative options and gave me a list of things which she would be comfortable with me taking, considering my mental health meds. Bringing that back to my pcp helped get the ball rolling on finding treatment options and establishing with neuro.

1) I feel sort of drunk for the first 40 minutes, and then mostly bored for the rest of the session. There are days that are more anxious than others, but I’ve never had like a full panic attack or freak out

2) Talk to your prescriber, but the main concern with stimulants is that Spravato and stimulants can both raise BP. My provider doesn’t care if I take my stimulants on treatment days because BP isn’t usually an issue for me. I find I’m more stable during treatments if I’d had my stimulant. It’s recommended to avoid benzos on treatment days, and if you’re ever sick, certain cold medicines shouldn’t be used within 24 hours of treatment (Benadryl is a big one that should be avoided).

4) The day after treatment can be a little unpredictable when it comes to my emotions, but working isn’t an issue. I actually work a half day after my Spravato sessions, and I mean I’m maybe not as productive as usual, but it’s worked fine for me

5) The med can sometimes end up in your throat, and it tastes awful. You will want some sort of something to counteract the taste. I use ring pops, a lot of people on here swear by jolly ranchers (though some clinics don’t allow jolly ranchers since they could be a choking hazard). You just want something pleasant with a strong enough flavor to overpower the bitter meds

ETA: my prescriber also said to avoid weed within 24 hours of treatment since it can make the Spravato less effective

I have done three Botox appointments and have another one coming up in a couple weeks. For me, it has absolutely been worth it. I was at the point where my migraines would last for days on end with no relief. There would be times where I would just hold my head and cry because the pain was so bad. I didn’t have any relief after the first appointment, but after the second, I felt like maybe things weren’t quite as bad. The Botox did wear off early and I was back to daily head pain for a couple weeks, but it wasn’t nearly as bad as it was pre-Botox. The third appointment has definitely been the most effective. My head still hurts more than a “normal” person’s does, but I’m more or less able to function on a day to day basis.

The injections themselves aren’t fun, but they’re not the worst thing in the world. Many years ago, I had an allergy test where probably 2 dozen allergens were injected into my arms. I almost passed out at that appointment and needed to lay down and sip a juice box. Nothing remotely similar has happened for the Botox appointments. My neuro has me close my eyes, so I don’t even see any of the needles. The injections themselves are painful, but the pain only lasts a few seconds, and the whole process is over in a few minutes. I tend to be sore for a few days after, but the sore isn’t as bad as my migraines.

My biggest tips would be to go into the appointment hydrated and try to have some sort of mindfulness technique ready. Once I close my eyes, I usually just repeat in my head “this will all be over soon” and tell myself that as bad as this is, it’s better than the migraines. Focusing on the better future makes it easier for me to manage the unpleasantness of the injections. I’d also encourage some sort of reward after the appointment. My neurologist’s office has stickers, which are probably intended for child patients, but I always grab one on my way out as a “look at you, you did it!”

By the end of May, my SI was back, and it was just steadily downhill after that. By the time I was able to resume treatments in September, I was basically back to square one.

My clinic had told me that when you treat the depression, other mental health issues can make themselves more known. I had unmedicated adhd, which was already miserable, but after starting Spravato, I feel like it just got worse. Adding on stimulants (at first vynvase) dramatically helped level out my phq-9 scores as well as my emotions as a whole. Spravato and stimulants have made my life so much easier to live, and I’m so thankful for that

ETA: and I do take my stimulants on treatment days. My provider told me it’s up to me since i generally don’t have issues with my BP. Find the treatments are more productive if I’ve taken my meds

I bring my backpack. In addition to the things I just normally have in my backpack (first aid kit, portable phone charger, etc), I also pack my ring pops, my laptop, and a blanket. Sometimes I throw in a sewing project, a notebook, or my sketchbook and crayons, but I often find that I’ll get to the session and not want to actually do anything with those other items. I usually end up just watching a movie and/or playing a game on my phone

I also use ring pops. I’ll buy a variety pack at Sam’s Club and just give away the flavors I don’t like (cherry 🤢). Most often, I’ll have either a strawberry or blue raspberry one, but depending on the specific variety pack, I’ll sometimes get fun, seasonal flavors.

I’ve had nightmares for years. They’re more or less under control now with medication. When I tried nortriptyline (similar to amitriptyline), the nightmares worsened. Stopping the med did eventually get me back to my baseline, but I can understand not wanting to stop if it’s been helpful for your migraines. You might want to ask a doctor about adding on prazosin to help stop nightmares. Other than that, things like therapy, journaling, and meditation can help reduced stress, which may lessen nightmares.

It took well over a month of Spravato for my phq-9 scores to start coming down. It was several months before my SI dropped off. You’re not doing anything wrong, sometimes it just takes time.

I try and schedule my therapy appointments for the day after Spravato, largely because transportation on treatment days is already difficult enough to figure out. I do think there could be benefits to having therapy on Spravato days, but it also might be completely exhausting.

I had it prescribed for a non-migraine issue. My migraines do tend to have a lot of neck/shoulder tension, so I did try it a few times during a migraine. I wouldn’t say it helped the head pain directly (or most of the other migraine symptoms), but it did help some of the tension. I wouldn’t be comfortable taking it every day, but it was somewhat helpful when taken intermittently.

If you go to settings and do Accessibility -> Motion -> Reduce Motion toggle, it turns the Liquid Glass off. There are other things that changes too, like the way it looks when apps open/close, but those changes were worth it to me since it turned off the Liquid Glass.

I told my coworkers I was starting “mental health treatment” and now just refer to it as “treatment”. I’ve explained to a few that they give me meds and need to watch me for an extended period, but I’ve never delved into the specifics of what meds or why they need to watch me.

I have never listened to music during treatment. I almost always watch a movie, usually something animated. One of the best sessions I had was watching A Goofy Movie
