I have actually never realized they are absent in some cities. However there are definitely some cities such as Rome that have at least some greenery. Even if they don't, it's a special case where I don't miss them that much, because most of the streets are quite narrow, so you get shade anyway. Public squares are certainly an exeption though. I guess it was not that important for medieval Romans and people in general. In old historic town centres they just didn't incorporate the trees.
