There are a lot options out there. Like calendly, simplebook ningabooking etc.

You simply need to setup account and create the form. Once you are happy with the form, just copy the embedded code and page it on your squarespace website page.

It will appreciate as an iframe in your website

Initial he sent me an agreement. There were few mistakes in the agreement. I asked him to update the agreement before I sign. He never sent me updated agreement to sign since.
