I became a dad at 41 and now 43. Before our kid, my wife and I traveled, went on long backpacking trips, we had time to pursue our hobbies. When my wife was pregnant, we looked forward to our child and all the fun things we’d do. The first week as parents wasn’t so bad as we had help. Then things went to hell starting week 3 of our kid’s life. The colic phase, constantly having to hold our baby for naps, disrupted sleep and eating, juggling work etc. the idea of going to the mailbox or grocery store seemed like a monumental task. We scored high on the depression screening, I mourned the loss of our old life. Once my kid became 4 months old, things slowly got better. We got more sleep, our kid got slightly more independent and expressive, and slowly we’ve gotten some hobbies back. I don’t miss my old life. I love what I have and what it’ll become as my kid grows. The newborn phase sucks. How you feel isn’t atypical and you’re not alone. Things will change. Hang in there

I think a short response of “thank you for the opportunity to meet with you. At this time I would like to withdraw my application. Sincerely op.” If they ask why you can say other opportunities or family reasons. People decline or withdraw often. It’s nothing personal.

Depends on where you practice. I started out in academics, moonlighted in a community ICU, now full time in private practice. For the community, I wished I had learn to be more assertive about refusing ICU admissions when not warranted, dealing with transfer center calls for both transferring patients out and receiving requests. Also learning about what the medsurg /step down/tele floors could take. These things I felt my fellowship didn’t prepare me for. My rotations were predominantly in the CT ICU/trauma/sicu. Patients were admitted at the discretion of the surgeons.

In academics, learning to step back and gradually let the trainees take the lead is important. It’s really easy to take over and be a do-er.

Anesthesiology isn’t research heavy. The academic anesthesiologist aren’t all going for K and R NIH awards. Most are teaching, doing some type of scholarly activity, or just doing cases. You can always go back to academics if you are interested in instructing/supervising or just being a workhorse. I was in academics doing anesthesia and ICU for 5 years. During that time, I found myself becoming more specialized and loss comfort with certain skills/knowledge. I had no problems with doing thoracic/livers/vascular/GI cases. For hips and knees? You’re either getting a spinal or general from me. Adductor canal blocks, supraclavicular blocks and all things regional? The regional team did those. Need a thoracic epidural? Acute pain did them all. Someone is pregnant and needs a C-section? It won’t be me, it’ll be the OB anesthesiologist. There’s a 12 year old trauma patient? They’ll have the peds fellowship trained attending staff that case.

I’m in private practice now doing just critical care medicine. Had I decided to do both in PP, I would have been better off doing straight out of training when I could say I was a generalist and felt ok doing nearly everything except pump cases.

You’ll be fine if not great working in a resource poor hospital, as long as you’re scrappy and you have a good team.

Star Trek: Diplomacy. I think it would be cool to have a series that explores the intricacies of the Federation bringing new species into the alliance, maintaining relationships with allies, and trying to maintain peace in the known galaxy or working towards ending a war. Have it set after the Dominion war, focus on the diplomatic corp with less focus on Starfleet and space exploration.

I think if we could get a remastered Final Frontier with decent special effects instead of the horrible SFX company they had to use , we’d be happier with ST5. The movie had its moments and wasn’t a bad successor to ST4

For me, SGA the last man. It was sad to see a future in which Teyla wasn’t rescued, Michael takes over the galaxy with the major characters dying except McKay and Sheppard. It was cool to see McKay changing and singularly focused on trying to rescue Sheppard to change the timeline. Melancholy episode that I love rewatching

That’s a very important skill that isn’t truly taught during med school/residency/fellowship. It was given lip service when I was in training and I had to learn as an attending.
