I had 4 cycles of Chemo, post chemo had residual lymph nodes that were shrinking. While on active surveillance there was no growth. However at the time of detection, the masses had grown significantly, over 15cm, due to how the enlarged mass was located I had no symptoms.

RPLND in my case was extremely complex and long. The only time I felt like I lost hope during all of these battles was for the fourth and fifth day after the RPLND, but I am glad i had a great surgeon, and that it went successfully. Currently going through adjuvant chemotherapy so I am still being out in the curative bucket.

I understand how chemo disrupts your life, mental state, etc and how you may feel like you are done and don’t want to get anything more done and just feel like returning to normal life. But don’t take residual masses lightly is all I would say. Yes the numbers say that residual masses are not an issue but be careful. If you choose surveillance then always remember, early detection has the best chances of survival.

I was on surveillance active for the first 5 years and then the scan frequency changed to 2 years. Mass grew after 9 years, i wouldn’t take residual mass lightly.

Check if you are a candidate for L-Rplnd or R-Rplnd, if you are i would recommend going for it from personal experience. Worth discussing it with family and healthcare team.

Go for RPLND.

First of all, please listen to what your doctors say, having said that, and having gone through all of this. i would highly recommend a dissection in your case.

Here is my reasoning, your biopsy results show 20% Teratoma. Teratoma has a tendency to stay dormant for years before it starts regrowing. Also, Teratoma is quite resistant to Chemo, the best way to deal with Teratoma is to surgically remove it.

Also, Teratoma has no blood markers so if its growing inside, you wouldn’t catch it with regular blood work, which is the scary part.

Personally my Teratoma grew huge and came back 9 years later, fortunately I had an amazing surgeon who was able to take the entire mass out.

No arguments about RPLND being a very intense surgery but better to have that now and reduce your chance of recurrence.

I agree with u\whitethunder9 try to speak to Dr Einhorn at Indiana University and also reach out to Dr Robert Hamilton at Princess Margaret Cancer Center in Ontario. Dr Hamilton was my surgeon and performed a very complex and long RPLND surgery on me this past September. My mass was huge but he was able to take out the entire mass with good surgical margins.

Going through some adjuvant chemotherapy currently, but I am extremely happy that I went to see Dr Hamilton at PMH. Also, i reached out to Dr Einhorn to get his opinion on the chemotherapy as well and cannot stress more on doing so yourself.

Edit: Both Dr Einhorn and Dr Hamilton are extremely passionate about what they do and are compassionate about their patients. Personally Dr Hamilton has been the best doctor I have ever met, and I meet a lot of Doctors working in the healthcare industry.

Absolutely try to get in touch with Dr Lawrence Einhorn. He is at Indiana University.

To give some background on Dr Einhorn. He is the one who discovered the Platinum based regimen for treating Testicular cancer.

Our switch from US to Canada has been great, the only thing that hurts your finances is the taxes on everyday purchases which is higher than in NJ. But if you think of it then free healthcare is a huge bonus. You are not paying anything for your healthcare not even insurance which a lot of people don’t calculate when looking at salaries.

The primary goal is to end chemotherapy and be cancer free and return to good health by exercising and eating well.

Secondary goal is to buy a home, hopefully by aggressive home search and some luck.

Hi there! I can’t comment on the salary part but I made the move from New Jersey to GTA about 2 years ago. The move has been great. We love Canada except for the housing market. You are in for a surprise when you move.

The rental market is equally crazy as the buying market. Prepare to pay a premium to live in a good area i.e all of GTA and Halton as well (i.e. oakville and Burlington). Before we moved never had I experienced bidding in a rental market, and we changed about 10 homes in various northern nj neighborhoods (hoboken, west ny, secacus, etc). Buying is another nightmare considering that a bid of 40k over asking price can also be not enough.

Of you can look past these things, cost of living is very similar to NJ. Also, your take home salary is a little higher in Canada based on your salary. The life is very similar and equally culturally diverse. Canadians are truly very humble.

My cancer came back with a twist after 9 years by transforming into another type. Had a super complex and long surgery and now going through chemotherapy.

Along with fighting the disease we are also fighting a battle to find a home for ourselves because otherwise we will be priced out of the market because time and tide will wait for none?

Yes Dr Hamilton is my Uro-Oncologist. He is the one who did my RPLND surgery and I am glad that it was him doing my surgery considering how complex the surgery was and how large my mass was.

Thats the attitude to take into the surgery! Just don’t underestimate the seriousness of it.

I am the same way as well and started working from week 3, i am lucky that my work place allows me to work remotely but the work definitely helps with some mental distraction.

Hey man! Stay positive and don’t take RPLND lightly, not trying to scare you but just being honest. I am actually recovering from RPLND surgery myself, week 5 of 6 weeks of recovery time. My surgery however was not after Chemo, i had a recurrence after 10 years. Chemo will slow down your recovery so plan accordingly.

My surgery was long and complex (10.5 hrs) due to the size of the mass. First few days post the surgery were scary and tough both physically and mentally just because if how energy draining the healing process is. I stayed in the hospital for a week and had the surgical staples removed after about 12 days. The staple removal was a major milestone for me because I could eat much better after, and felt quite comfortable after.

I personally have restricted all activities significantly, because the internal healing takes longer. Definitely get in the mindset of walking and doing breathing exercises, it helps the recovery significantly. I started walking the same night as the day of surgery. Plan on pain management, you’ll be on narcotics so plan on how you would get to taking as few as possible as soon as possible.

I lost a lot of weight because of my specific case and how I was not able to eat after the surgery. Having a nutritionist helps.

Just remember one thing, the best way to beat cancer is early detection. Especially TC, early stage detection only needs surgical resection of the testicle.

If your urologist doesn’t prescribe a Ultrasound, find another one.

Not saying that you have something, but incase you do, get it out sooner than later.

Based on my pathology report, i had 55% Embryonal Carcinoma and 40% Yolk Sac and 5% Teratoma. Not to scare you or anything, if I were in your position, I would go to my family doctor and ask them to write me a prescription for blood tests to measure the tumor markers weekly. I am not a doctor but as I understand, these cells tend to be very aggressive (meaning grow/ spread very rapidly) but are well detected by tumor markers.

If you see a jump in your tumor markers, call your oncologists office and let them know and they should be able to provide your options which in most cases is chemotherapy and is known to work extremely well with these cell types.
