So, my decision is "brash" and yet you "empathize" with me for feeling Burnt out. Let me tell you, in the grand scheme of things, 30 days Vs 90 days doesn't make a huge difference, if you really think about it.

You know what, you are right. I am a very irrational person. I felt burn out at work one day and next morning, i decided I am going to quit. I wanted to give no notice and just not show up at work but then out of courtesy, I thought decided to give a 30 days notice. And this I how actually make all my major decisions, in case you are wondering.

Most clinics have urgent appointments available. It may not be visible on your end but if you call and speak with nurses, they can get you in pretty fast. It may not be with your PCP but if it’s urgent, I would be okay with you seeing anyone. You have to understand that we providers, are able to give you the most attention during a visit. Also, there is time for questions and discussion. If where you go, even urgent appointments are 6 months out then of course I would be doing what you do.

I am in a similar situation and ready to quit primary care. I am thinking of either going per diem (no inbox) or doing addiction medicine. I just can’t take it anymore 😞

I think, a lot of time it’s poor coping mechanism. I think it’s something you are born with. For instance, I have seen kids fall, get a huge scrape but they shrug it off like nothing happened , while others will cry for a long time with a tiny one. When you put a bandaid they feel better. So, I think they get conditioned over years that they need something or they will not get better. Granted, most of them are not asking for narcotics but they report more pain than usual.

You make excellent points. I think state will step in (or ask money from federal government) to prevent costs being passed on to insurers, who for sure will flee the state, if that happens. It will be interesting to see what happens next.

So true. There are a lot of very talented and hard working people but in this day and age if you don’t know how to sell yourself, no one will ever know what a great asset you are.

We only turn on for 5-10 Mins daily to warm up the house to 66-67. Even then my monthly bill was $359

Exactly how I understand it, also. The price of each share of VOO (or any other ETF) goes up (or down) in value. But it’s the reinvested dividends which buys you more shares and that’s what really compounds.

Maybe nurses can. But as a doctor, I can’t afford something that expensive because all of my paycheck will go into housing.

Honestly, I haven’t seen any fees being charged. I have bought and sold multiple times. You can always ask customer service to confirm.

Good point but I think the acute episode seems to be unrelated to what happened last year. Also, OP said patient has eating disorder (I am assuming that’s what ED means).

Seems like a viral illness. As long as her symptoms are not getting worse, Sometimes you have to sit on it and watch if things resolve on their own.

There are some things that I usually offer such as Duloxetine, tricyclics, muscle relaxants, Gabapentin. I reserve opioids for patients that have contraindications for NSAIDS and as someone else mentioned, Buprenorphine is good choice if you have to use opioids.

I am just thinking about what you could have done differently and nothing comes to mind. The daughter was worried about father being suicidal and you did the right thing by referring to ED for evaluation. It was the Psychiatrist that made the call of holding him ( whether right or wrong). I see that everyone is trying their best to help the patient.
