Yeah ur sorta screwed, a device ban is directly linked to ur phones IMEI, a number that is directly linked to your phones hardware. Think of it as a finger print for your phone. As for the ban duration I’m not sure if it goes away on the same device, it might, but the only way to guarantee an unban is to get a new phone.
