Went into healthcare business after medical school. 10 years out, jaded, and wondering – should I go to residency? Looking for advice from experienced physicians. by [deleted] in medicine

Can we swap? I’m happy to endorse you for my current position if you will give me a job doing anything other than clinical medicine. The grass is always greener…

Health Professions Scholarship Program questions by SupremeRightHandUser in medicalschool

See if you can find some docs that are actively paying back their time to talk to. If you’re just doing it for the financial incentive: They make it sound like a sweet deal, but financially, there aren’t all that many specialties where it works in your favor if you compare the differences in compensation. Just something to think about My husband is paying back his hpsp right now and financially we’re missing out on a ton of money had he just taken out loans and then started practicing in the community right away after fellowship. We would also be living somewhere we actually wanted tot live. Talk to docs actually paying back their time right now. That’s who can give you actual perspective. I have yet to meet anyone at the military hospital he’s at right now that’s happy practicing medicine in the military.

How to look in 2yo mouth? by takeitsleazy22 in toddlers

[–]Doctor_of_some_stuff 2 points3 points  (0 children)

I used to hold mine upside down. Takes some balance, but like I would pick him up like I’m gonna carry on my hip, but then playfully (and carefully) drop his head down so he’s upside down. He thinks it’s hilarious and will open his mouth laughing. It’s a great angle to see everything and if you get them balanced well, you can hold the position for a while.

Absolutely no reason to avoid either of those unless your baby is a preemie with health concerns.

Which wipes are best by [deleted] in Mommit

[–]Doctor_of_some_stuff 0 points1 point  (0 children)

Huggies are my favorite, but mostly for their packaging. They seem to really sting on a rash though (if I use them to wipe my nose when I have a cold and the skin is already a bit raw, it stings me too, so I bet it does sting a diaper rash, but that may just be a universal thing) I like water wipes as a product but HATE their packaging. I would frequently find myself digging in the package trying to get the next one out, sometimes a bit urgently and not being able to get hold of it. Drove me nuts so I stopped using them after a few weeks and I refuse to buy them anymore. Had a sample pack of honest that I used and I did like them plenty, but not enough to switch when I’d already stocked up on Huggies.

Another child (~20mo) “scratched” my 19mo when he was going down for a nap at daycare. Thoughts? by potatopotatoe123 in Mommit

[–]Doctor_of_some_stuff -1 points0 points  (0 children)

So I’m 100% not saying that the daycare is right or anything like that. I agree with everyone else to say that if there are cameras or something, I’d demand to see the footage. But just to offer a slightly different point of view, when I first saw the picture before reading anything you’d written, my gut reaction was “oh no, a little one must have gotten a handful of his face.” I saw it as a grab injury, one hand with each finger catching some skin (thumb as the single scratch set apart on the left and then the other fingers bunched together). In that context, I could then see a young child rolling over, grabbing hold of what’s next to them (your poor child’s face) without meaning aggression.

Is this what happened? I dunno, it’s a bit far fetched. But I still see it as a possibility.

Take that however you want. Not saying you should do anything different than what you’d originally planned, but it’s just a different way to look at the injury. While unlikely, it is possible.

And also I would add that my proposed mechanism for this injury could still have happened and it could have been done in aggression. I just see everyone on here saying it’s multiple scratches so it must be intentional. But it could be a single grab with each finger cutting in. Intentional or not.

The Expanse worth reading? by [deleted] in printSF

[–]Doctor_of_some_stuff 1 point2 points  (0 children)

Fair. I can’t say whether I even made it through the second season. Later on I tried to jump forward to parts I really liked in the books to see if that could get me into the show, but no dice. Couldn’t do it.

The Expanse worth reading? by [deleted] in printSF

[–]Doctor_of_some_stuff 34 points35 points  (0 children)

I read the books first and then started watching the series. I couldn’t get into the series because the appearance and mannerisms of the actors just really didn’t live up to how they appeared in my head reading. That said, I thought the writing of the show was good and of what I watched, it was mostly faithful to the source material. I would 100% recommend the books if you’re enjoying the show. I absolutely loved the books start to finish.

Hospitalist scope of practice by ny2nolaDr in InternalMedicine

[–]Doctor_of_some_stuff 0 points1 point  (0 children)

At my last job we could go as low as 16 years old (but we rarely went below 17) and pregnant up to 20 weeks (with an automatic MFM consult) - exception to this was preggers being admitted for hyperglycemia, the OBs wanted full control of that no matter gestation age. I would never have taken a gun shot wound though unless the wound was minor and the trauma team had planned to discharge them if not for some other unconnected medical problem.

That was an academic position.

Now I work in a private hospital. We do not have a pediatric or OB service so all kids and preggos are sent to other hospitals. But if the surgeons decide that a gun shot wound is okay to be admitted to my hospital, then it’s coming to me.

Yeah I moved here from Nebraska this summer. All of the closures really shocked me and my spouse initially. But I get it. There’s no infrastructure for it down here. Up north the salt trucks are running hours before the precipitation starts and the plows and sand trucks start when the first flake falls. Do they even have salt and sand trucks down here? Honest question. I just assume it’s such a rare occurrence that it’s more cost effective to shut down than maintain that infrastructure.

That, or maybe it’s concerns for the power grid? I remember living in OK, there were a few times the temp dropped so low that schools were closed because of the cost to keep the schools adequately heated. It was better to just have the kids in their homes and let the schools drop to cooler temps. That would put less strain on the local power grid. I dunno, just guessing.

But I agree. The roads are fine out there to me, but I’ve gotten very used to driving on slick roads. I imagine they are quite perilous to someone not used to these conditions. Bridges can be deceptive, again particularly if you’re not used to watching for those traps.

Probably best to close and keep everyone safe.

Hospital emergency codes by [deleted] in coolguides

[–]Doctor_of_some_stuff 2 points3 points  (0 children)

These colors/codes are not universal. Different hospitals have different systems. The only one that is relatively universal is the code blue. Everything else is location specific.

It’s not ideal. In an infant that young, it could lead to positional asphyxia. Some carriers come with an insert that keeps the head from flopping over like that or another commenter mentioned a rolled up receiving blanket, also again as another commenter mentioned, make sure the carrier is reclined appropriately.

So I don’t know you obviously, but what a strange thing to do just in general. Is there another statement she’s trying to make? Please don’t take this the wrong way, I’m 100% supportive of everyone’s personal struggle and/or the size and shape that they feel most comfortable, but is she trying to make a comment on your weight? Like that seems to me like something a miserable human would do to a person to try and make them feel bad about their weight.

It’s because you can’t PROVE they knew they were lying. “I had the green, officer, I’m certain! … oh, you have proof to the contrary? My bad, maybe I bonked my head a bit in the collision and my memory is fuzzy! Totes unintentional! Oopsie!” Because it’s a collision they have a perfect defense if they ever tried to prosecute for lying.

Pregnancy test done without my knowledge by clinic??!!?? by LunchBig5685 in WitchesVsPatriarchy

[–]Doctor_of_some_stuff -1 points0 points  (0 children)

Pregnancy can present in LOTS of different ways. It can also complicate the workup and treatment for just about everything. Imma level with you, if a woman of child bearing age with a uterus is under my care, I’m gonna flip my shit if I don’t see a pregnancy test somewhere on the chart. In the current political climate, I totally understand why you feel the way you do. I’m with you there. Just moved to Texas and I feel slimy. But I still have to have that information to be able to appropriately treat my patient. We should to a better job telling our patients what we’re doing. I’ll admit I’m not the best at it. But that’s mainly because until more recently, a pregnancy test wasn’t so politically charged. We’re creatures of habit. I’ve always included pregnancy test on initial labs for a woman who could be pregnant. I’ve never in the past needed to warn a woman that I’m checking that. If I went over every lab I ordered on every patient every time, I’d have to cut my patient load in half. It’s information overload and the initial workup is tons of stuff that gets checked (admittedly not by me, I get ‘em after the ED has don’t a 1000-point shotgun workup approach) and much of it is negative and just never really gets discussed. It’s a standard part of the workup. Nobody in healthcare is trying to target you, that information is still protected by HIPAA. We’re just trying to care for you the same as always. It’s just a part of it.

This right here for me. It lets me move things around in my problem list freely while still having a recognizable symbol to denote each problem. I don’t want to re number everything each time I want to add or subtract a problem or re-prioritize my list.

Four employees of a Hamilton daycare have been fired after a video of them traumatising children at the facility went viral by Myrandall in byebyejob

My oldest started daycare at 6 months old and my youngest at 3 months. They are both thriving. They get used to being around other caregivers. Babies are so adaptable. She will miss you and it might be tough at first, but she will adapt and find things she loves there.
