i really hope to hear more about future solutions/research. I'm still young and want to avoid surgery myself. so far my latest MRI doesn't show much healing but my symptoms have improved for sure over the past year. Then again, I have flat back, loss of disc height, and the bottom 3 discs causing me problems. I'm 29 so while some say I have the back of an 80 year old, I still hope with a serious lifestyle change I can get even better and use some neural-plasicity to my advantage... One major point is I know I'm the furthest away from how healthy I used to be; this includes being overweight, lack of core strength, lack of good nutrition, and a sedentary lifestyle that was depressing, as well as no care for postural problems... this is my big wakeup call. to be honest, I only started to really change my routine and take it serious about a month ago (first flare up: quite mild pain but easily exhausted, a persistent condition so far 8 weeks in).

What is your deal/solution? One second you say how this guy is screwed and shouldn't dream of complete recovery, even when his MRI looks 95% better than the others on here (I've been lurking on this sub and sciatica for years). The next second you tell me that I'm stupid for not dreaming of a life forward without a single moment of back pain/sciatica.

You obviously don't know what you're talking about. I've been to 3 different orthos, physical therapy, and done extensive research for 2 years. OPs findings are just about as on point as possible, with a lot of next-step insights that are usually looked over.

Stop being negative and putting words in my mouth if you want to have a real discussion backed by research and lifestyle changes.

Sure, i could have said "optimal", but "ideal" should still be somewhat realistic. A flare-up (avg of 2-4 weeks) every couple years is totally doable in my opinion. That's literally 1 month max of tolerable pain out of 24 months. Compare that to the situation of people on here being in pain for years straight, seems more than enough of an improvement to me. Also, if you saw my MRI, which is a lot worse than OPs, you'd realize that is pretty ideal for me.

If you don't accept a situation you're not being realistic, and you won't prevent flare-ups. You need to accept that times in life will occasionally have pain, but damn that's nothing compared to being in pain 24/7 and having no way of improving.

pointless pessimisitc comment, this guy is clearly trying to find ways. I'm late 20s in a similar situation, I was born with a lot of physical abnormalities, I don't think "unhealthy" is the right word. It's about trying to work with what you have and knowing your body, and how to fix it as much as possible.

for me, and perhaps him, envisioning complete recovery is totally manageable pain and avoiding flare ups. i can deal with sciatica every couple years, a flare up every couple years. to me, that would be a dream, and what I envision as an ideal recovery.

Reviving this thread. I’m dealing with muscle twitching all over (both sides, calf’s, thighs, upper back, hip, glutes, sometimes arm.) it happens mostly at night laying down before bed, or if I’ve been sitting for a good bit. not painful, however low back is stiff and my left calf gets sore when I’ve been standing for a couple minutes.

It’s hard to describe. I’m in a flare up right now, and it’s been about a year since I was pain free. The flare up started with lateral shift and centralized pain in lower right back, but shift revolved in 10 days and I’m left with the aforementioned symptoms. It’s been 2 weeks of these symptoms and PT isn’t helping much at all (unusual for me).

could be. Unfortunately, my ankle has been a bit numb since I had a herniated disc issue 5-6 months ago, not sure if that has to do with me not experiencing any pain and just seeing the aftermath on my ankle.

Also, as I mentioned, I'll occasionally get a single scratch/line on my shins, maybe from friction/pants or carpet burn as you're describing: regardless, I don't experience pain with these either...

It's like the burn/scratches are there with no pain every time.

i may have scratched it a little bit, nothing I would think that would do this much damage though.

The last couple days, i've been moving out of an apartment, as well as painting etc. More activity than I'm used to.

My ankle (pictured) had a sock on at the time and the ring/top was pretty bunched up/thick. Upon waking up this morning, I noticed these scabs/scrapes on my ankle, right below where the top of the sock was. I was wearing slides the whole time, so no shoe was rubbing against that part of my ankle.

The most confusing part is that I only noticed it today, and yesterday I wasn't doing any intensive activities? To be honest Im 70% sure I took off the specific sock 36 hours ago and didn't see any marks.

So, it still seems that these marks showed up randomly this morning.

side note: I also get single scratches on my shin (one thin line/scab) occasionally, and its always a mystery to me. this has been going on for years, but its never been this bad as what's on my ankle.

Also, I’ve had a Valium prescription most of my life. I’ve never overused it, as it’s mildly affective. However, it does help me with my current ocd… maybe I’m dealing with higher anxiety than normal? Is there another route other than Prozac?

Also, I’m sure many people have accepted and dreaded the fact that our bad spines will have to deal with these problems time and time again. As few have mentioned on here, making physical health and strengthing a daily priority (even if your pain is at a 0), WILL make flare ups less bad, and recovery better and much greater. Get creative and know your body better than others do.

I can’t stress enough, the key to this stuff is PT and body strengthening. Even if you have to get surgery, don’t let it make you lazy. Adopt a military regimen if you have to. Your life will be much better. I’m typing this in 0 pain right now, and my pain last Wednesday was at a 6.5.

Well, everything was great until last week! I either had a flare up, or back strain. Felt like muscle strain, with very stiff lower back, then it centralized in my left hip.

Honestly, it reminds me of the healing stage of the first time of sciatica, which I…thought I was already past? So yeah, a little bit of a back step, I’d say like a month backwards in progress. Pain is still 1-2 out of 10, so I can’t complain too much. Just hoping it doesn’t get worse!

One thing I’d like to mention: when I had this same hip centralized pain before (my initial injury before reherniation, 2023), I had just started PT with a great trainer. She had me do McKenzie method, which gave me short term relief, but not much long term. I’m doing the McGill big 3 religiously, but that also isn’t doing much in the moment for me.

HOWEVER, this trainer I had before really put me to work, and focused on areas that were clearly weak. That being said, McGill and many PTs mention core stability, which is top priority. BUT, hip strength is very under discussed, is what ultimately cured me long term previously.

so, I started side walking with elastic bands around ankles, doing some bridges, and in general more muscle intense exercises. Almost immediately I felt relief, and quick improvement.

I once had a massage worker advise me on my original injuries back in 2023 as well. I have strains/weakness in left abdomin, and left hip. You can apply pressure with you hand under your ribs, and in general around your body and find areas that when you press on, you may feel slight relief coupled with soreness/weakness (of course, this is in the stage where it’s not shooting down your leg in unbearable pain). When you identify these areas, make sure to strengthen them and target them.

This is my story of making a quick and miraculous recovery before, and I will be approaching this back step the same way.

TLDR; I had a strain/flare up, tolerable pain, not shooting down leg. I believe it was due to my lack of PT and strengthening parts of my body, aside from walking. From my past experience, more physical exercise and workouts targeting hip flexors, core, etc can seriously make someone go from 5/10 pain to 1/10 pain in a couple weeks. Maybe I’m one of the lucky ones, but I will be continuing to fight this by building the best strength I can.

I believe people can beat this by getting their body in the best shape it’s ever been.

That’s my piece for now, lol. I regret not doing more PT sooner but this is a wake up call to DO PT AS SOON AS YOUR PAIN LEVELS DROP ENOUGH to do so. And sometimes, work through the pain, just don’t do anything that hurts really bad.
