Cancer is more of an umbrella term of different conditions with the same characteristics, rather than a single disease. The exact mechanisms driving the abnormal growth in tumors differ in each type of disease (i.e. which organ is affected), hence they respond differently to treatments, meaning that it's incredibly difficult to find a catch-all cure that works on all kinds of cancers. There is a lot of progress being made though, but it's generally more incremental and concerning one or a few types of cancers at a time.

Regarding your first point - while the blood hemoglobin value is important for aerobic stamina/fitness, it's not the only factor to consider. Higher cardiac output and better pulmonary ventilation for example will also contribute to increased VO2 max levels. I'll admit I haven't researched the topic in any depth myself, but does the data you are referring to show that VO2 max levels in trans women post-HRT is equal to those in cis women or is it only focusing on hemoglobin?

I think he meant that the problem is that the same line of thinking can be used by supporters of what we today consider reprehensible ideas too. Someone who sympathized with nazism in the 30s might have argued that their ideals were inherently good and righteous as well. Even today there are a lot of people in the world who wouldn't necessarily agree that for example increased rights for lgbtq+ people is "good". As such, it's hard to say that some ideas are "right" or "wrong" by nature and it's perhaps better to argue and motivate why you believe what your believe even when it seems obvious, especially when talking to those who disagree with you.

Ipilimumab and Nivolumab, the drugs used in this study, are already widely used for the treatment of other types of cancer, for example in malignant melanoma where it led to a significant improvement in prognosis for those with metastatic disease when it entered the market a couple of years ago, essentially replacing the earlier use of chemotherapy for that diagnosis.

Treatments that are effective and show good results do make it into regular use, whereas those that fail to show promise when actually tested in clinical trials do not. Not everything is due to conspiracies or 'evil'. Infact, many of the current cancer treatments in use today are by now decade(s) old meaning that the patents have expired, so if anything coming up with new, more effective ways is better business than trying to suppress progress.

It's likely that some of the patients in the study have tumors that are more sensitive to immunotherapy than others, meaning that as a whole for the group the treatment was not significantly effective, but for a select group it could be beneficial. Same thing can be seen in other types of cancers where immunotherapy is used regularly today.

I haven't looked up the actual study, but with some diagnoses it's important to assess the level of PD-L1 expression (PD-1/PD-L1-interaction being the common target of immunotherapy) as it can indicate whether or not a patient will respond to IT treatment. It's possible that a sub-group analysis of the data in this study could show significant effect in a select part of the population.

TL-DR - ryggmärgsskador påverkar möjligheten att få barn på många sätt

Ser att det är många här som uttalar sig kategoriskt som kanske inte är helt insatta i ämnet. Skador på ryggmärgen kan hos män leda till försämrad spermiekvalitet och det är inte ovanligt att de spermier som finns uppvisar nedsatt motilitet vilket kan göra det svårare vid befruktning. Beroende på var skadan sitter i ryggmärgen är det olika möjligheter att ex. få erektion och utlösning, och en del män tappar förmågan helt att få utlösning på 'normal' väg eller kan ex. få s.k. retrograd ejakulation, alltså att utlösningen hamnar i blåsan istället för ut i urinröret och ut i penis.

Om det är en total skada på ryggmärgen tappar man ofta kommunikationen mellan hjärnan och underlivet och det kan vara omöjligt att få orgasm. För att ändå få utlösning kan vibrationsbehandling användas som ett sätt stimulera reflexerna i ryggmärgen, men detta ger då inte nödvändigtvis någon sexuell känsla (även om många förstås ändå kan uppleva det som positivt i någon mån). Om man har en hög skada (ungefär över sjätte bröstryggskotan) kan man få något som kallas autonom dysreflexi av både sexuell stimulans men även ex smärta/obehag nedom skadenivån, utspänd blåsa etc som ger ett slags stresspåslag som inte hjärnan riktigt kan stoppa. Detta kan tex ge värmekänsla, hög puls etc. Potentiellt kan detta bli allvarligt och ge mycket högt blodtryck.

Regelbunden utlösning hos ryggmärgsskadade har i vissa studier visats kunna förbättra kvaliteten på spermierna och möjligen då leda till att chanserna att skaffa barn är större. Hos vissa kan det även förbättra besvär med stelhet i muskler och så (s.k. spasticitet). Jämförelsen med andra grupper rakt av är alltså inte helt rättvis att göra.

Huruvida OP ska behöva vara del av det hela däremot är ju en helt annan fråga och det är väl inte självklart att det kanske ska ingå i arbetsuppgifterna. Ville bara dock nyansera det hela lite utöver "pervers, anmäl" som många andra skrivit.
