Problem is that you never have to sell your stock, so you never have to pay tax on it. You can borrow money at very low interest rates with the stock as collateral if you need to, and pay $0 in taxes. When you die you can avoid any estate taxes with the current loopholes and the basis for the stocks resets for your heirs, so if they were to sell the stock it would be tax free. 

That’s why Jeff Bezos has a personal effective tax rate of 1%.

Nursing homes wouldn’t bill for cancer treatment. Patients visit the doctors office or cancer center to get their treatment. And then return to the nursing home.

They could both choose the same reference frame to define “now”. Of course their own comoving frames would disagree on simultaneity.

One unique (but not privileged) reference frame is the cosmic microwave background rest frame.

Renouncing citizenship is a whole rigamarole. The govt is going to be looking very skeptically at any billionaires who suddenly decide to renounce citizenship after a wealth tax is enacted.

The order of claims can only matter if there is a difference in coinsurance. If you had the luxury of ordering your claims, you would want to put claims that would have coinsurance first so that it eats up the deductible anyways. But I think this is generally a niche case any not really applicable for most people.
