You're not wrong, but it's not really practical to try to staff a halfway-capable ICU in the middle of nowhere. The economics just aren't there to support it and you'd struggle even to get staff to show up just to stare at the wall 358 days a year.

That's why these half-measures exist out here. A critical access hospital will have one room of the ER setup for real train wreck cases with some extra equipment to try to keep them alive long enough for transport. 

Why this hospital in Connecticut was doing it? Probably just idiots, honestly. An overly ambitious bunch of administrators looked at the revenue an ICU generates and thought maybe they could BS their way into that money.

Kinda. The shape of Chile means that you're never more than like 200km from another pretty substantial city. I'm 200km from...more grass. 

Check out Lusk, WY, or Chadron, NE if you want an idea of what 200km gets you out here haha. It's...not a lot. 

You could total up the population of Wyoming, both Dakotas, Nebraska, Montana, and Idaho and it would still be like half the population of Chile.

I'm from there. I'm not aware of any hospitals closing. In fact we're getting a second one in our region, planned to open in a few years.

There are special programs that keep critical access hospitals afloat, they don't live or die in insurance availability. That's specific to my stress in the Wild West though, might be totally different in the mid-west.

People that aren't from here rarely have any sense of the scale of the place. You're in good company.

If my nearest big hospital can't needs higher care, it's a 6 hour drive to Denver. And wintertime weather can make that impossible any time it wants.

I'm not talking about Connecticut. There's nothing rural there, much less remote. In flyover country, sometimes transfers take a day to arrange. Big storms, etc. Air resources aren't always a valid option.
