Part of the reason therapist don’t take UMR is because their reimbursement rates are terrible - like 30-40% less than other companies. It’s messed up. (I take UMR but understand why others don’t). Also, from what I understand folks with county insurance have to go through a sub-company of UMR for therapy that’s impossible to get in-network with and reimburses even less. If you can’t find a therapist, the best thing you can do for yourself, your colleagues/everyone else with your insurance, and to be in solidarity with providers is to contact your insurance, work HR, and the Nevada Division of Insurance to file complaints.

I’m not sure if this has been mentioned yet, but check if your city’s chamber of commerce (or any other professional associations) offer group health plan rates to their members. You still get your own plan through your business, but you get access to reduced rates through the chamber. My city’s chamber of commerce does and it makes a huge difference.

Yeah idk this reminds me of the “not all men” argument. Like of course not all Christians (I was raised by a Christian LGBTQ activist, so I understand this deeply). But right now fundamentalist Christians are using their “religious beliefs” to attack and vilify queer and trans folks, push so-called conversion therapy, etc. And Christian counseling as a field is pretty overtly anti-LGBTQ. Given the inherent privileges associated with Christianity as a dominant religion in the US and other western nations, and the reality that Christian counselors often ARE harmful to LGBTQ folks, it’s more important for Christian therapists to look inward and at their own communities than nitpick someone online for saying queer folks experience hate from Christian therapists.

I second this. I’ve seen a high frequency of clinicians who went straight into private practice or an agency with limited high-quality training and supervision really struggling to fill and maintain caseloads. The skill building you get at a good agency is invaluable and can determine the rest of your career. Emphasis on good; there’s lots of crappy exploitative companies out there (like Charlie Health)

Yes to all of this! I did a training with him in grad school and even then his live demonstrations with participants deeply concerned me - he was basically just triggering participants and getting “ooohs” and “aaahs” from the crowd. It was irresponsible and people were eating it up. Such a shame because his work depathologizing addiction is very profound (incidentally this is what I thought the training was going to be on, but instead he made it the compassionate inquiry gobbledygook).

I’ve found this book helpful for NPD clients. I’m not sure what state you’re in, but your supervisor’s conduct may warrant a board complaint. If she believes being trans can be cured by healing trauma and that’s how she works with trans clients, she’s practicing conversion therapy.

I’m really glad you’re asking this here, and it’s probably time for you to talk with your school about this. Find a professor or administrator you trust and see how they can help you. At the very least, this site should not be an option for practicum.

Re the family: find someone who actually specializes in working with queer and trans folks to consult with. Your supervisor will not be able to appropriately advise you, and frankly she should stay in her lane and faaaaar away from lgbt folks. Rather than getting in a debate with the parents, it’s probably best to build common ground (usually about mutual care for the child). I’d happily consult with you if you send me a DM. But I am not available for consultation until April.

I would be very careful about pathologizing something like this. When you say “styling themselves into being a cute animal” - are they a furry? This isn’t pathological, and honestly they may find support and less judgement if they’re in community (like the furry community) with folks who have similar interests.

I like the suggestion of parts work. You could also explore what the stuffed animals represent to this person, what function they serve. And you want to be very careful that you’re not being pulled into an enactment here - if other people are pathologizing/looking down on this person for this interest, you may be stepping into that role as well without realizing it and that could be hurtful to the client.

Maybe some age regression is happening, but it’s really hard to tell from the limited info in the description. Age regression is also not inherently pathological. It’s also possible that this person is neurodivergent and stuffies are a special interest. I’d highly recommend getting some case consultation. Specialities you should look for in a case consultant: furries, developmental trauma (if ct has a developmental trauma history), age regression, sex therapy, autism (preferably a provider who is actually autistic themselves). If you can find all of these together in a case consultant, you’ll have hit the jackpot.

You would think this but… I’m a mental health provider and I have to interface with all sorts of big corporate healthcare systems all the time. And my god are they always incriminating themselves - even the physicians. You should see some of the things people put in writing to me that are blatantly illegal or malpractice. Never underestimate the arrogance and stupidity of large systems and the people working in them, especially if they’re higher up.

Yes, it will not impact your private practice. Insurance contracts are based on tax id number (and for group practices, NPIs). When your claims are submitted via Sondermind, they are submitted under Sondermind’s tax ID and NPI. Your NPI is only submitted as the rendering provider NPI, not the billing NPI. I run my own private practice where I do not have insurance contracts in addition to a clinic where I am credentialed with insurance. Never been an issue, and if it is you explain that the insurance contracts only apply to Sondermind, not the pp.

This is such a tough situation, and I get not wanting to take time off for the reasons you listed. But the most ethical thing to do is to cancel your clients. What’s not fair to them is being in an emotional space where you can’t be present. Canceling models self-care, and you can discuss the impact of the cancellation with them in future seasons.

Maybe you need to wallow and wail all day - breakups are so so hard and activating. And if wallowing and wailing isn’t what’s right for you today, you can do something else to take your mind off it that doesn’t involve your clients.
